SUMMARY
Under the direction of the IFS/ECM Program Director, the Enhanced Care Management Supervisor (ECM Supervisor) ensures the quality of services provided by the Lead Care Managers (LCM) and oversees the day to day needs of the LCMs. The ECM Supervisor also provides direct service carrying a small caseload providing all the duties of a Lead Care Manager, addressing the clinical and non-clinical circumstances of high-need clients enrolled in Medi-Cal managed care. The ECM Supervisor also serves as an advocate for clients, an active member of the interdisciplinary team, a liaison with other programs, and external health and social service providers in the community.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Provides guidance and staff development training for ECM LCMs, including new hire trainings.
- Ensures that ECM strategies and services are whole-person centered, linguistically, and culturally appropriate.
- Guides LCMs in steps to ensure the client’s safety and provides immediate consultation and guidance for any situations needing escalation of care.
- Immediately brings complex or high risk situations to IFS/ECM Director’s attention for consultation and direction.
- Take the lead on troubleshooting issues with different Electronic Health Records, attend trainings, keep up to date on requirements and changes issued by the different Managed Care Plans, and run monthly reports.
- Monitors continuous quality improvement measures for coordination of care through documentation review, clinical consultations, peer review, and other related activities to ensure quality of services.
- Oversees charting and documentation standards adhering to ECM contracts with Managed Care Plans (MCPs).
LCM DUTIES:
- Collaborates and communicates with clients, caregivers/family support persons, and other providers to promote wellness, recovery, independence, resilience, and empowerment, while ensuring access to appropriate services and maximizing client benefit.
- Assess client needs in the areas of physical health; mental health; SUD; oral health; trauma- informed care; social supports; housing; vocational/employment; wellness; and referral and linkage to community-based services and supports.
- Oversees the development and implementation of the Individual Assessments/Care Plans.
- Offer services where the client lives, seeks care, or finds most easily accessible, including office-based, telehealth, or community-based services.
- Connect clients to other social services and supports that are needed (e.g., community support group).
- With permission, coordinate/advocate on behalf of client with health care professionals (e.g., PCP).
- Utilize evidence-based practices, such as Motivational Interviewing, Harm Reduction Techniques, and Trauma-Informed Care principles.
- Work collaboratively with hospital staff or other institutions regarding Transitional Care Planning.
- Conduct outreach and engagement activities to facilitate linkage to the ECM program. Outreach and engagement includes phone calls, mailed information, community visits, and presentations.
- Accompany clients to office visits, as needed and appropriate.
- Evaluate progress and update goals.
- Provide mental health promotion.
- Arrange transportation, as needed.
- Complete documentation within timeframes established by the Managed Care Plans.
- Attend weekly staff/team meetings and supervision.
- Attend training as assigned (e.g., MI, ACEs Certification).
- Collect and enter client data as requested.

Qualifications
MINIMUM REQUIREMENTS
- At least 3 years of relevant work experience
- Associate’s Degree
- Multilingual capabilities, especially Spanish/English
- Valid CA Driver’s License
- Must be 21 years of age or older
- Ability to complete the training program and ongoing educational requirements
- Ability to work flexible hours, including occasional night/weekend work
- Access to personal transportation for home and community visits
PREFERRED REQUIREMENTS
- Long-time resident of the community with good knowledge of community resources.
- Prior experience as a Community Health Worker, Peer Support Specialist, Medical Assistant or similar role
- Experience using Electronic Health Records
- Bachelor’s Degree
- Experience supervising or being in a Lead role
