Description
ID#C22013A - Assists facilitators in their work with Child and Family Teams by collaborating in the development and arrangement of specialized services as specified in the individual Family and Child Service Plan. Provides direct one-on-one behavioral intervention, and skill development to children and their families based on specific behavioral crisis intervention plans, within their scope of practice.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Develops and maintains a resource and contact list. Links families to network providers and community resources and follows-up on all referrals. Assists with developing new resources for families.
Responsible for recording progressing meeting goals of treatment plan. Trains the child in strategies and techniques to deal with specific problems in daily living and coping. These may include such problems as anger, social isolation, and aggression.
Provides services where and when the child and family require them as contained in the Child and Family Service Plan. This may be at home, school, community, or elsewhere as planned and required.
Transports child and family as needed. May also transport groups of children of families to programs and services as needed.
Provides in-home and in-community services to families. May include but not be limited to completion of forms, coaching on contacting services, training in daily living, etc. Assures safety. Participates in Quality Assurance Activities.
Qualifications
Bachelor’s degree in psychology, social work, or related field preferred and one (1) year experience in providing direct services to seriously emotionally disturbed children in a school, residential, day treatment, mentoring, shadowing, or similar setting; OR
Two (2) years college and four years experience in providing direct services to seriously emotionally disturbed children in a school, residential, day treatment, mentoring, shadowing, or similar setting; OR
High School Diploma and five (5) years experience in providing direct services to seriously emotionally disturbed children in a school, residential, day treatment, mentoring, shadowing, or similar setting.
Basic knowledge of social learning principles and positive intervention.
Department of Justice clearance (Child Abuse Index).
Culturally sensitive and competent.
Possess valid Class C California Driver’s License and driving record must meet SFVCMHC, Inc’s insurance requirements.
