Description
ID#31213A - Provide psychosocial services, such as therapy, assessments, case management, and crisis intervention.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Participates in the assessment and evaluation of seriously and persistently mentally ill (SMI/PMI - Serious Mental Illness/Persons with Mental Illness) adults. Determine individual’s strengths, needs, and resources. Assess client’s level of psycho-social impairment, self-care potential, family and support network availability, independent living needs, financial status, educational, employment or training needs, and social recreational needs.
Develop a written, comprehensive service plan, which specifies the treatment services, activities, and assistance needed to accomplish the objective identified between client and case manager.
Under the direction of the Program Manager, the Mental Health Clinician (case manager), will advocate for the clients with community resources, mental health providers, hospital and other agencies serving the chronic mentally ill (CMI = chronic mental illness) population to assist client in assessing needed services. Participate in provider or inter-agency meetings as assigned by the Program Manager.
Assists clients in developing and maintaining independent living skills, including social/recreational and vocational or educational goals, needed to implement and achieve goals of the individual service plans.
Performs intakes and initial assessments.
Assures that planned services are provided.
ADDITIONAL DUTIES AND RESPONSIBILITIES:
Completes documentation per Center and DMH (Department of Mental Health) standards within mandated time frames.
Provides individual, collateral, and group psychotherapy as required and/or planned.
Provides crisis intervention as needed.
Collaborates with parent partners, psychiatrists, and network providers.
Other duties as assigned.
Qualifications
The ideal candidate must have a master's degree from an approved graduate school or program; AMFT = Associate Marriage Family Therapy or ACSW = Associate Clinical Social Worker; Experience working with adults in a mental health setting; Valid CA Driver License and insurable by the Center’s automobile
Must maintain current status as a licensed professional under California law.
At least one (1) year experience working in a mental health setting with CMI = Chronic Mental Illness population.
Must have a valid California Driver License in good standing and personal, valid insurance. Must be insurable by the Center’s auto insurance carrier.
Must act in accordance with all Health Insurance Portability and Accountability Act (HIPAA) of 1996 and related state law confidentiality requirements. Must successfully complete the Center’s HIPAA training, pass the HIPAA test, and receive the Certification of Compliance.
Must have personal, reliable transportation to conduct field visits.
