Description
ID#3612A - Under the supervision of the Program Manager, the Mental Health Counselor II functions as a member of a multi-disciplinary team, which provides field-based services. The role of the team and the Mental Health Counselor II is to coordinate and provide engagement and outreach activities, mental health and rehabilitative services, targeted case management services, and crisis intervention services to all FCCS clients. Provides ongoing support to enrolled clients in the community in order to decrease hospitalization, incarceration, unemployment, homelessness, and help clients successfully transition into adulthood.
RESPONSIBILITIES :
Provide individual, group, and family counseling, as needed and within their scope of practice.
Function as part of a multi-disciplinary field-based treatment team. Identify and mobilize both family and community resources in order to meet the needs of clients. Provide intensive service coordination in order to enable clients to function at their highest level of independence within the community.
Assist clients in developing and maintaining independent living skills, social / communication skills, and coping skills.
Participate in the assessment and evaluation of seriously mentally ill adults. Determine the individual's strengths, needs, and resources. Assess each client's level of functional impairment, substance use / abuse history, potential for self-care, the availability of family / significant other (s) / caregivers / social network, independent living needs, financial status, educational, employment or training needs, and social / recreational needs.
Qualifications
The ideal candidate must have a Bachelor's Degree in the field of social work, psychology or related field. Current NPI number.
Minimum of two (2) years experience in a mental health setting. Case Management experience necessary. Knowledge and experience in providing assertive outreach to clients and an ability to work with multi-ethnic and multi-cultural clients is necessary.
Must have access to a personal vehicle, have a good driving record and be insurable by the Center's automobile insurance carrier. Attend the Center's Driver Safety course.
Bi-lingual / bi-cultural in Spanish, Russian, Armenian, or Farsi is a plus.
