Description
ID#35149A - The Mental Health Counselor II provides a range of services as part of the treatment team. Mental Health Counselor acts as a case manager and may conduct individual/group rehabilitation, collateral services, and linkage to formal and informal resources.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Works with the Therapist, Family and the treatment team to develop an individualized Family Plan taking into account each family’s strengths, goals and needs.
Assists families in obtaining appropriate services within their communities.
Assists families in obtaining appropriate activities to strengthen family relations.
Is flexible and creative in providing support and services to families in their home, at school sites, in agency offices, or other off-site locations as outlined in the Treatment Plan or as needed by the families.
Models appropriate social skills for family.
Carries as caseload of CFSP families providing ongoing assessment of the family’s basic needs as well as the assessment of needs for services.
Provides and/or arranges transportation services for families as appropriate.
Schedules intakes and completes all appropriate documentation needed for opening cases.
Responsible for documentation including Progress Notes/ Unit of Service logs, Case Management Needs Assessment, relevant portions of DMH and agency’s documentation.
Participate in weekly staff meetings, Treatment Team meetings clinical update meetings, family meetings and individual and group supervision sessions.
Assist with crisis intervention as needed including being available to families after hours and on weekends when appropriate
Is respectful and sensitive to each family’s cultural and individual needs and customs as appropriate.
Is expected to become familiar and establish relationships with community agencies and service providers in order to be aware of possible resources available to CFSP families within their community.
Complete all documentation in a timely and comprehensive manner.

Qualifications
Bachelor's degree in human services or related field or equivalent, bilingual, and valid ID and at least 2 years of experience working with SED children and their families. Bilingual (Spanish)
Strong writing skills, organizational skills, computer skills. Knowledge of community resources.
Possess valid Class C California Driver’s License and driving record meets SFVMHC, Inc. insurance requirements.
