Description
ID#36152A & ID#36151A - Provide direct and indirect job placement services, mental health services, substance use disorder treatment, system navigation services, legal services to address barriers to re-entry, and linkage to medical care for communities disproportionately impacted by past federal and state drug policies.
POSITION RESPONSIBILITIES :
Administer a comprehensive Whole Person Care assessment at time of program intake.
Participate in assertive assessment and evaluation of clients referred from community partners. Determine individual's strengths, needs and resources. Assess client's level of psychosocial impairment, self-care potential, family and support network availability, independent living needs, financial status, educational, employment, and social / recreational needs.
In conjunction with client, create and implement a thorough service plan incorporating strengths, barriers, and short / long term achievable goals of treatment.
Assist clients in developing independent living skills and resources including social / recreational, vocational, legal, or educational goals, needed to implement and achieve goals of the individuals' service plan.
Provide referrals to and facilitate linkage to medical care providers that have established partnerships with the Center.
Provide clients with referrals and direct linkage to legal services providers that can assist with legal issues impacting client's reentry to society / workplace.
Assist clients to acquire skills and tools for managing stress / trauma to experience improved daily functioning and emotional wellbeing.
Create and maintain case management notes in EXYM from client meetings, track referrals and linkage attempts made, record client progress toward treatment goals along with results of client pre and post self-assessments.
Complete all documentation within mandated time frames and in compliance with county, state, and Center guidelines.
Transport clients to and from appropriate service in company vans as necessary.
Provide crisis intervention - intervene with the client / others at the onset of a crisis to provide support and assistance in problem resolution and to coordinate or arrange for the provision of other needed services.
Will work as part of a multi-disciplinary team; participate in staff and clinical meetings and community meetings as assigned.
Qualifications
Education :
Bachelor's degree in the field of Human Services, or related degrees with minimum of two years' experience in counseling preferred.
Experience :
Minimum of two years' experience in mental health setting with mentally disabled population preferably with homeless individuals, conducting individual and group counseling.
Bilingual Spanish preferred.
Chemical dependency / Mental Health services experience preferred.
Must have a good driving record and must be insurable by the Center's insurance provider.
Be comfortable working with a target population of homeless, justice involved, mentally ill, and those suffering from substance abuse and other debilitating conditions.
