Program Summary:
STAHR (San Francisco Transgender Alliance for Health Resources) is an innovative, collaborative, client-centered Health Access Point (HAP) program specifically focused on addressing the needs of trans women in San Francisco (SF). STAHR operates through a close working partnership that brings together two highly respected SF agencies and programs, all of whom have distinguished and extensive experience in reaching, serving, and retaining trans women in HIV, HCV, and STD testing, care, and support. STAHR strives to reach and serve all trans women living in SF, with a strong emphasis on African American and Latina trans women.

Role Summary:

The Prevention Case Manager will work closely with the Program Manager along with the Harm Reduction Specialist and the Outreach and Community Engagement Specialist and will be responsible for overseeing Case Management for clients in the STAHR Program. 

 

Key Responsibilities:

  • Conduct outreach and recruitment activities to deliver peer-based HIV prevention services in venues where transgender communities congregate and socialize including city-wide events, clubs, bars, support groups, etc.
  • Perform quality data collection, data entry, maintenance, tracking of client enrollment/activities, and timely submission of units of services for monthly, quarterly, and yearly reporting requirements. 
  • Enroll clients in various medical care services including primary care, mental health services, needle exchange and syringe access, gender affirming care, and safer sex and substance use
  • Provide supportive services and resources to trans clients at Trans:Thrive Drop-In as well as inside various SF jails, psychiatric facilities, and rehabilitation sites.
  • Develop curriculum and facilitate educational workshops, social support and retreats.
  • Promote and represent agency services at outreach events
  • Participate in trainings, monthly staff, program, and department meetings, agency events, and necessary HAP meetings for SFDPH.
  • Manage case management services of STAHR program by conducting intake, assessment, and re-assessment of eligible transgender clients
  • Provide case management services to ongoing caseload and develop individual-client centered goals, resource and care navigation, harm and risk reduction counseling, and treatment plans following agency’s standards of practice.
  • Provide comprehensive linkage support, education, shelter, and treatment regarding PEP/PrEP, DoxyPEP, STD/HIV testing, substance use, harm reduction, access to basic need services, and primary medical care. 
  • Create new and innovative support groups for trans sex workers and trans women of color. 
  • Other duties and responsibilities as assigned.
  • This position requires working Saturdays and may require working late nights and/or Sundays for pop-up testing events.

 

Minimum Qualifications: 

  • One year of college or equivalent combination of education and experience.
  • Three years of experience working or closely associating with transgender individuals and issues.
  • Familiarity with issues and concerns of transgender sex workers and substance users; experience working with multiply diagnosed patients (HIV, mental health conditions, and substance abuse), sexual and gender minorities, immigrants and refugees. 
  • HIV/Hep C Testing and counselling certification.
  • Experience working professionally and ethically within multi-racial and multicultural settings.
  • Ability to work independently as well as within a multidisciplinary team.
  • Demonstrated excellence in attendance and reliability, interpersonal, organizational, verbal, and written communication skills in English
  • Detail-oriented with strong follow-through skills, including attention to accuracy and deadlines.
  • Consistently available to work weekends, evenings, and non-traditional work hours.
  • Fully vaccinated against COVID-19, complete with booster shot(s) and able to show proof of vaccination.
  • Up to date with vaccinations, especially seasonal flu vaccine.
  • Proof of TB clearance (issued within the past 12 months).

  • Bilingual English and Spanish.
