SFDPH strives to achieve its mission through the work of two main Divisions - the San Francisco Health Network and Population Health.Positions in class 2586 Health Worker 2 may be located at the following locations : Zuckerberg San Francisco General Hospital (ZSFG), Laguna Honda Hospital (LHH), various clinics, and other City Departments.Shift : Days, evenings, weekends, and / or holiday shifts may be required.Under supervision, the Health Worker 2 performs a wide variety of paraprofessional duties in a service program, and functions as a liaison between community residents and program staff; provides counseling and advice to patients regarding health problems; may supervise Health Worker I, and may drive or accompany patients between their homes, hospitals or other social agencies. The essential functions include, but are not limited to the following : Participates in the health service training program;Provides information and resources to patients regarding health care and other facilities;Assists patients in utilizing services;Makes follow-up contacts when required;Serves as liaison between the professional staff and the community;Assists in gathering and evaluating data concerning the program;May perform incidental clerical duties;May transport ambulatory patients;May provide language assistance;May pick up and deliver supplies and equipment.The 2586 Health Worker 2 may perform other related duties as required / assigned.Working Conditions : Some positions may require employees to work directly with individuals within locked jails or psychiatric facilities; the homeless population and individuals who have or are carriers of infectious / communicable diseases.How to qualify Experience : One (1) year of verifiable experience within the last five (5) years, performing a combination of at least two (2) of the following duties : Serving as a liaison between targeted communities and healthcare agencies;Providing culturally appropriate health education / information;Providing referral and follow-up services;Providing informal counseling, social support and advocacy;Escorting and transporting clients;Performing pre-clinical examinations.Substitution : Possession of a Community Health Worker Certificate can substitute for 6 months of experience.Special Conditions : Some positions may require additional special condition(s), which may include : Bilingual Proficiency;Experience providing health services to children;Homeless Programs Experience.Desirable Qualifications : Various lived experiences including incarceration, homelessness, and / or substance use.Selection Procedures : Candidates must complete all subsequent steps to advance in this selection process.Important Note : Please make sure it is absolutely clear in your application how you meet the minimum qualifications.CONDITION OF EMPLOYMENT : All City and County of San Francisco employees are required to be fully vaccinated against COVID-19.The City and County of San Francisco encourages women, minorities and persons with disabilities to apply.#J-18808-Ljbffr
