INTAKE COORDINATOR
The Intake Coordinator position conducts initial screenings and assessments on potential clients to determine mental health needs. Provides outreach services to maintain or when appropriate expand client census. This position supports the STRTP Program.
Responsibilities:
STRTP Mental Health Intake
- Review referral packets received by stakeholders.
- Respond to referrals received via email or phone within 72 hours.
- Conduct pre-placement interviews.
- Complete and distribute acceptance or denial letters to referring agencies.
- Coordinate and transport minors for placement on the intake date.
- Complete all steps from admissions checklist
- Complete and distribute staff intake consultations for STRTP clients.
- Complete all intake paperwork and distribute in accordance with the admissions checklist
- Complete referral form and submit to billing department.
- Complete enrollment of client into Electronic Health Record System (EHRS).
- Enter referrals into Electronic Health Record System as applicable.
- Provide daily census population report to LA County Probation by Noon. Update supervisor the status of referrals on a consistent basis
- Attend Interagency Placement Committee screening meetings as applicable.
Outreach
- Monitor and maintain the client census average of 10.
- Perform the outreach activities necessary to identify potential new sources of referrals and clients.
- Drive to various sites throughout California and conduct interviews of potential residents, families, and/or agency representatives.
- Conduct an initial screening of potential STRTP referrals, evaluate degree of “fit” with the current STRTP population, and share impressions with supervisor, STRTP Director and STRTP Head of Service.
- Attend scheduled county screening committees and represent San Gabriel Children’s Center, Inc. in a positive and professional manner.
- Consult with program staff to anticipate possible discharges; conduct proactive searches for potential clients.
- Notify placement agencies in Los Angeles and the surrounding counties in writing of STRTP vacancies on a weekly basis.
- Assess potential residents’ dangerous behavior prior to intake and facilitate the completion of the San Gabriel Children’s Center, Inc.’s Initial Safety Checklist.
- Obtain appropriate written authorization prior to placement of a resident at SGCC, including obtaining Acknowledgements of the SGCC Privacy Policy from the legal representative of each potential resident.
- Follow the San Gabriel Children’s Center, Inc.’s Intake Procedure including obtaining necessary signatures and informed consents for San Gabriel Children’s Center, Inc. to provide STRTP and mental health treatment services.
- Complete the Patient Financial Information (PFI) Form.
- Notify referral agency in writing of SGCC’s decision regarding acceptance or denial of admission and explain the rational for such decision. Notification letters are subject to prior approval of the Vice President, Programs or his/her designee.
Requirements:
- Bachelors degree program in counseling, psychology, or social work.
- At least 1 year in the social work, counseling field, or STRTP experience.
- Maintain a valid driver’s license and provide proof of insurance. Must have held a valid driver’s license for a minimum of 5 years.
- Obtain, within 30 days of employment, Pro-ACT, Red Cross CPR and First Aid Certification and maintain such certification current through the term of employment with SGCC.
- Must be able to obtain and maintain Live Scan clearance, pre-employment physical, TB test and drug test.
- Bilingual Spanish preferred.

San Gabriel Children’s Center has embarked upon implementing and embedding Trauma Informed Care (TIC) principles throughout the organization. Candidates should be aware that all employees are involved in the implementation and the practice of Trauma Informed Care within the organization.
