What are the responsibilities and job description for the LVN CASE MANAGMENT ASSISTANT position at San Gorgonio Memorial Hospital?
POSITION SUMMARY:
The LVN Case Management Assistant provides hands-on post-acute care planning services to our hospitalized and emergency department patients, as assigned by an RN Case Manager or Social Worker, in accordance with department policies and procedures. The incumbent will also support the RN Case Management staff in level of care screening according to department policy and procedures.
REQUIREMENTS:
Education:
- Some college education preferred. Graduate of a school of vocational nursing.
Licensure:
- LVN required.
- BLS required.
Experience:
- Case Management Experience preferred.
Other Skills:
- Must have excellent clerical and organizational skills.
- Superb verbal and written communication/people skills in person and on the phone.
- Must be familiar and comfortable with medical terminology.
- Computer skills and proficiency in Microsoft Office.
- Strong analytical thinking and the ability to manage multiple tasks concurrently.
- Excellent customer service skills to provide ongoing support.
- Able to manage and deal with stressful situations.
