Working within a skilled nursing facility (SNF) environment, you will assess residents, develop individualized treatment plans, and collaborate with other healthcare team members to support optimal outcomes. This flexible, as-needed role is ideal for a licensed speech-language pathologist looking to make a meaningful impact. Key Responsibilities :
- Assessment & Diagnosis : Conduct thorough assessments of residents' speech, language, cognitive-communication, and swallowing abilities.
- Treatment Planning : Develop and implement individualized treatment plans to address identified speech and swallowing disorders, adjusting as needed based on resident progress.
- Therapeutic Intervention : Provide direct speech and language therapy, as well as cognitive and swallowing therapy, to support residents' communication needs.
- Documentation : Maintain accurate, up-to-date records of treatment plans, resident progress, and care modifications in accordance with facility and regulatory guidelines.
- Family & Staff Education : Educate residents, families, and staff on techniques to support effective communication and safe swallowing practices.
- Collaboration : Work closely with other rehabilitation professionals, nursing staff, and healthcare team members to provide coordinated, comprehensive care.
- Regulatory Compliance : Ensure all therapy services are delivered in compliance with federal and state regulations, including SNF-specific standards. Qualifications :
- Master's degree in Speech-Language Pathology from an accredited program.
- Current licensure as a Speech-Language Pathologist in California.
- Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred.
- Previous experience in a skilled nursing or long-term care setting is highly desirable.
- Strong communication and interpersonal skills to work effectively with residents, families, and the healthcare team.
- Knowledge of electronic medical records (EMR) and proficiency with documentation. Physical Requirements :
- Ability to perform physical tasks such as standing, walking, and assisting residents as needed.
