YOUR DAY AS OUR MENTAL HEALTH CASE MANAGER:
Every day as a Mental Health Case Manager, you'll step into the lives of individuals and families in need, acting as their advocate and guide. You'll deliver direct case management, teach essential life skills, and document progress as you help clients navigate life's challenges. Your work will include arranging transportation, referrals, and education while building a network of community support to empower each client's journey. By fostering relationships with community agencies and presenting our mission, you'll serve as a vital link between San Luis Valley Behavioral Health Group and the broader community. Your efforts will echo far beyond the walls of our nonprofit human services organization, creating ripples of positive change.
QUALIFICATIONS FOR SUCCESS:
- Bachelor's degree with 3 years of experience in the human services field OR a master's degree with some experience in human services preferred
- QMAP certified
- Experience providing case management services
- Ability to apply crisis intervention skills
- Ability to observe, report, and keep a record of client symptoms and progress
- Ability to work both independently and as part of a team
- Physical ability to perform light physical work
- Basic computer skills, with proficiency in Microsoft Word, general Windows products, emails, voicemail, and electronic record systems
- Excellent communication skills, both verbal and written
- Creative problem-solving skills
- Valid driver's license
