Position Overview:
Under the supervision of the Director of Community Development and Programs, the outreach worker will provide culturally sensitive HIV field outreach services to identify individuals who are living with HIV/AIDS but have not entered into the system of care, to assist individuals at risk of HIV/AIDS in becoming aware of their HIV status, and to assist HIV individuals in accessing the continuum of HIV care through facilitated referrals.
Knowledge/Skill Requirements:
- High school diploma with equivalent combination of two (2) years HIV education and experience in community education.
- Some experience in HIV/AIDS related medical/social/testing/prevention issues, including addiction, homelessness, and sexuality.
- Good communication, interpersonal, organizational, and time management skills.
- Ability to help clients with diverse psychosocial needs.
- Motivation and self-direction. Ability to prioritize competing responsibilities in fast paced clinical care setting.
- Ability to work autonomously and as a member of a diverse team.
- Sensitivity to ethnicity, culture, gender, sexual orientation, values, beliefs, and behaviors.
- Flexibility with client's priorities, evolving needs, and goals.
- Ability to meet deadlines.
- Computer literate in a PC environment including the ability to enter and access data relevant to program required to include software data systems
- Effective verbal and written communication skills.
Job Duties:
- Conduct community HIV and Hepatitis C screenings. Conduct pre/post HIV counseling and testing services utilizing standard testing procedures.
- Maintains accurate & confidential records of all testing information.
- Provides follow-up services for testing recipients in the form of post-test counseling, disclosure of test results and refers as appropriate.
- Deliver outreach services where communities of at- risk populations may gather; this includes preparation and distribution of educational, promotional and recruitment materials.
- Actively seek and recruit persons diagnosed with HIV; engage clients into integrated HIV Primary Care and Behavioral Health services, as needed.
- Act as a resource to provide clients with other health and social supportive services as needed.
- Collaborates with other HIV / Hepatitis C counselors/testers and community development team members.

Training:
- The outreach worker will be responsible for attending all mandatory training including evidence-based approaches to self-management support, proactively addressing the needs of patients and families in this community, culturally competency, effective communication, care coordination, OSHA, HIPAA, Risk Management, and HIV/AIDS etc.
Additional Requirements:
- Must have a valid driver license, reliable transportation, and current vehicle insurance.
- Must be willing to work at different SMF locations if requested.
- Ability and willingness to work flexible hours, including evenings and Saturdays, as required.
