Responsibilities :
- Oversee the daily operations of the nursing center Social Services Department in accordance with the National Association of Social Workers (NASW) Code of Ethics.
- Maintain compliance with federal, state, and local regulations, policies, and standards of care for specialty practice.
- Promote a positive environment that encourages patients / residents to achieve their individuality, independence, and dignity.
- Ensure the highest level of physical, mental, and psychosocial well-being and quality of life for patients / residents.
- Supervise and support two or more Social Services staff members.
- Accountable for the Social Services practice, measuring outcomes to interventions, and managing departmental productivity.
- Complete psychosocial assessments for each patient / resident to identify their social, emotional, and psychological needs and strengths.
- Establish and document discharge goals for each patient / resident at the time of admission.
- Provide education to the staff regarding the importance of Social Services and the psychosocial needs of patients / residents and their families, particularly concerning aging and disability issues.
