Home Health Nurse/Case Manager

Job Summary
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health team. The ideal candidate will provide high-quality patient care in the comfort of patients' homes, ensuring their health and well-being. This role requires strong clinical skills, effective communication, and the ability to work independently while adhering to established protocols and standards.

Responsibilities

• Conduct comprehensive assessments of patients' health status and needs in their home environment.
• Develop, implement, and evaluate individualized care plans in collaboration with patients, families, and healthcare teams.
• Administer medications and treatments as prescribed, ensuring adherence to safety protocols.
• Monitor patients' progress and adjust care plans as necessary based on clinical findings.
• Educate patients and their families about health management, disease prevention, and self-care techniques.
• Maintain accurate documentation of patient care activities in compliance with HIPAA regulations.
• Coordinate with interdisciplinary teams to facilitate effective patient service and discharge planning.
• Provide support for geriatric patients, including those transitioning from a Level II Trauma Center or outpatient services.

Skills

• Proficient in medical terminology to effectively communicate with healthcare professionals and document patient care accurately.
• Strong patient care skills with a focus on empathy and respect for patient dignity.
• Excellent patient service abilities to foster positive relationships with patients and families.
• Knowledge of HIPAA regulations to ensure confidentiality and security of patient information.
• Familiarity with managed care principles to navigate insurance processes effectively.
• Experience in outpatient settings is a plus, particularly with pediatric intensive care unit (PICU) cases or geriatrics.
• Ability to perform discharge planning efficiently, ensuring smooth transitions for patients returning home.

Job Type: Contract

Pay: $27.00 - $30.00 per hour
