The Sangamon County Department of Public Health is seeking applicants to perform duties primarily as an Early Intervention Service Coordinator. These duties include:

• Attend seminars, workshops, and educational trainings related to early intervention for professional growth and grant requirements
• Coordinate services and support efforts in a timely manner to families who are within the early intervention process
• Develop and aid family with individualized family service plans (IFSP) that meet family’s priorities and strengths
• Develop and maintain strong relationship with families within the program offering transition options and assisting in the development of a transition plan
• Maintain extensive documentation and ensure timely submission of required paperwork
• Provide service coordination ensuring all qualified children under age 3 receive ongoing early intervention service delivery at the Child and Family Connections site for Region

• Schedule and maintain intakes, evaluations, and assessments with eligible families

The work assigned to this position may not include all possible tasks in this description and does not limit the assignment of any additional tasks for this position.

Required Qualifications:

• Bachelor’s degree in human services, behavioral science, social science, or a health related field with experience in a community based social services program o OR a current State of Illinois registered nursing license
• A valid Class D Illinois Driver’s License
• Applicants must be a resident of Sangamon County or be willing to move into the County within 6 months of employment.
• Ability to use personal automobile with proof of current insurance
