The Sangamon County Department of Public Health is seeking applicants to perform duties primarily as an Early Intervention Service Coordinator. These duties include:
- Attend seminars, workshops, and educational trainings related to early intervention for professional growth and grant requirements
- Coordinate services and support efforts in a timely manner to families who are within the early intervention process
- Develop and aid family with individualized family service plans (IFSP) that meet family’s priorities and strengths
- Develop and maintain strong relationship with families within the program offering transition options and assisting in the development of a transition plan
- Maintain extensive documentation and ensure timely submission of required paperwork
- Provide service coordination ensuring all qualified children under age 3 receive ongoing early intervention service delivery at the Child and Family Connections site for Region

- Schedule and maintain intakes, evaluations, and assessments with eligible families

Required Qualifications:
- Bachelor’s degree in human services, behavioral science, social science, or a health related field with experience in a community based social services program o OR a current State of Illinois registered nursing license
- A valid Class D Illinois Driver’s License
- Applicants must be a resident of Sangamon County or be willing to move into the County within 6 months of employment.
- Ability to use personal automobile with proof of current insurance
