Sangre de Cristo Community Care is looking for a Full Time Registered Nurse to join our dynamic home health team in Walsenburg, CO. Serving a variety of patients across Southern Colorado, including pediatric patients and those with developmental disabilities, Sangre de Cristo Community Care is known for delivering exceptional high-quality compassionate care with a strong team of local health care professionals, including skilled nursing, physical therapy and occupational therapy, providing a seamless opportunity for high-quality hospice, palliative care and home health services to residents in southern Colorado.

This is a Full Time Benefited position. Monday - Friday, 8-5

Responsibilities:

· Plans, coordinates, implements, and evaluates the nursing care of all patients.

· Utilizes clinical knowledge and skills to identify patient needs and coordinate services and referrals to meet these needs

· Works closely with the inter disciplinary team to provide a continuous appropriate, comprehensive, and responsive plan of care for patients.

Requirements

· Must be at least 21 years of age.

· A valid non-restricted RN license in the state of Colorado is required.

· Must have a valid non-restricted driver’s license.

· Flexibility and availability to work a weekend schedule if needed.

· At least 1 year of home health or hospice experience preferred.

Salary : $35 - $38
