Position Overview
As an Admissions Coordinator, you will play a crucial role in facilitating the admissions process for patients seeking hospice, home health, or palliative care. You will be the first point of contact for families, guiding them through the admission procedures with compassion and understanding. Your role is pivotal in ensuring a smooth transition for patients into our care.

Key Responsibilities:
· Ensure effective communication with marketing, field admissions, referral sources and Interdisciplinary Team.
· Take referrals from all sources for Hospice, Home Health, and Palliative patients, not limited to phone orders, including MD’s, SW’s, case managers, patients, and family members.
· Educate patient/caregiver of hospice/home health/palliative services.
· Obtain written consent from patient, family, or patient assignee when criteria for hospice/palliative admission are met.
· Complete prior authorizations when needed.
· Provides accurate and appropriate documentation of patient/family services.
· Facilitates Hospice/home health/palliative referral process, serves as a resource to review and consult available patient information, to determine hospice care needs and criteria; coordinates and communicates with Hospice agency and referral sources to promote care coordination and a timely admission process.
