POSITION SUMMARY
The Central Navigator – Consults Team Member assures that all the appropriate clinical, insurance and coding information is collected for inpatient care. Inpatient care includes patients who are seen by Sano Specialty Care providers at our inpatient hospital systems. This includes consults and on-call surgical procedures. The Central Navigator – Consult Team Member assists physicians and clinic resources in completing all the necessary processes for inpatient care according to established guidelines. The Central Navigator provides accurate information and maintains open communication with patients, providers, and facilities about updates or changes.
PRIMARY RESPONSIBILITY
- Obtain patient demographic information from hospital partners on patients seen on consultation by Sano Specialty Care providers
- Create patient charts in eCW.
- Create encounters in eCW to reflect completed consultations.
- Obtain consultation documentation and operative notes from hospital EMR systems and import into clinic EMR (eClinical Works)
- Data Entry into electronic medical record system Communicate with patients as an extension of the clinic team as it relates to consultations
- Book follow-up and post-operative appointments as needed
- Help coordinate with outside providers to obtain prior medical records as needed. (i.e. Primary Care Physicians, Cardiology, Pulmonology, etc)
- Communicate with patients, providers, and other parties
- Answer phones and direct patient calls as needed
- Ensure accuracy of data for surgical procedures for OBERD, Patient Reported Outcomes Measures
- Communicate benefit coverage and insurance eligibility with patients regarding their follow up care
- Any other job duties that might be requested by the doctors or management to help the office run efficiently
SKILLS
- Understanding of Internal Classification of Diseases (ICD) and Common Procedural Terminology (CPT) coding
- Proficient knowledge of medical terminology
- Strong customer service skills
- Electronic medical record experience (eClinical Works preferred)
- Highly detailed and accurate, with excellent organizational skills
- Excellent communication and interpersonal skills
- Critical thinking and problem solving skills
- Excellent skills in Microsoft Office (primarily Word, Excel, PowerPoint and TEAMS).
- Must be computer literate with the ability to learn new software applications

EDUCATION AND QUALIFICATIONS
- High School diploma or equivalency
- 1-2 years of administrative or patient experience
- Knowledge of general administrative and clerical procedures
