POSITION SUMMARY
Clinical Navigator (CN) is as a clinical resource that is responsible for assisting the medical provider in patient treatment issues, providing direct patient care, and maintaining proper standards of quality and integrity. The CN will perform clinical duties while acquiring supplementary knowledge of the medical field and helping make the workplace an efficient and effective environment for all. The CN will support varying clinics throughout the week to ensure adequate clinical coverage and patient care.
PRIMARY RESPONSIBILITY
- Optimize and nurture the patient experience within Sano by navigating them through their journey with us.
- Perform appropriate patient care duties, including but not limited to :
Room patients in clinic
Scheduling follow up appointmentsPrepare and record charts appropriately within the EMREmploy skill in patient care and communicationRemove staples and stitchesAssist with preparing and administering injectionsPrepare for and assist physician with minor proceduresCommunicate with external healthcare providers (Physician, PA, NP, ATs, PTs, OTs, RNs) on patient statusPrepare and assist patient with needed paperwork (e.g. work status forms, school notes, etc)Patient educationSend in patient prescriptions as directed by physicianUtilize company Inventory Management SystemAssist physician with EMR functions required for proper documentation and capturing appropriate dataCoordinate advanced imaging and other outside referrals as neededCommunicate with the patient and surgical facilities to schedule surgery procedures. Send any associated orders for procedures.Monitor patient and staff communication daily (Klara / TEAMS)Complete Family and Medical Leave Act (FMLA) and Short-Term Disability (STD) paperworkReport MRI and lab results to patients as directed by physicianObtain prior approval from insurance companies on prescriptions, injections, or procedures as neededServe as preceptor and educational mentor for students and residentsFacilitate the front office as neededHelp manage physicians day-to-day scheduleHelp answer patient communication when not in clinicAttend mandatory full staff and clinical meetings and acknowledges receipt of and reviews minutes if absentAdhere to all company policies and proceduresEngage primary care offices, athletic trainers, physical therapists, and other referral sources in an effort to increase awareness in the communityEnhance the experience of referring providersPerform other related duties as required or assignedSKILLS
Understanding of Internal Classification of Diseases (ICD) and Common Procedural Terminology (CPT) codingProficient knowledge of medical terminology and anatomyStrong customer service skillsElectronic medical record experience (eClinical Works preferred)Highly detailed and accurate, with excellent organizational skillsExcellent communication and interpersonal skills; expresses thoughts and ideas clearly, both verbally and in writing; establishes trust and confidence quicklyExcellent skills in Microsoft Office (primarily Word, Excel, PowerPoint and Teams).Must be computer literate with the ability to learn new software applicationsCOMPETENCIES
