Hickory Falls Health and Rehab is seeking a Social Services Assistant to join our team of dedicated health care professionals.

- Schedule and manage long-term care plan meetings with residents and their families
- Conduct assessments for residents as required by CMS regulation
- Assess nature of patients' situations by interviewing and reviewing personal history
- Develop and execute individual treatment plans/care plans
- Communicate patient updates to all relevant parties
- Participate in meetings to alert appropriate staff members of relevant social work items, room changes, and discharges
- Maintain knowledge of Medicare & Medicaid regulations to ensure services are in compliance with professional boarding standards, state and federal regulatory requirements.
- Skilled Nursing Facility/Long Term Care experience is preferred
- A Social Work degree is preferred
- Possess the ability to deal tactfully with personnel, residents, visitors, and the general public
- The ideal candidate should have a passion for working with seniors, a high degree of customer service and extensive marketing and sales experience
- Compassionate and caring demeanor
- Excellent written and verbal communication skills
