Position Title: Community Health Planner
Department: CHR/Diabetes Program Director
Salary Grade: NE5 Per Class Code
Position Status: Full-Time
FLSA Status: Non-Exempt

Position Summary
Under general supervision of the CHR/Diabetes Program Director, The Community Health Planner will be responsible for planning, developing and implementing activities and initiatives that support existing and emerging health needs in the Pueblo. Key activities and initiatives include coordination of the Santa Clara Health Committee; ongoing assessment of community health needs; facilitating the development and implementation of Community Health Improvement Plans to address priority needs; community engagement; preparing and submitting related reports; event coordination and collaboration with internal and external partners.

Essential Duties and Responsibilities
Coordinates the Santa Clara Health Committee and subcommittees, including scheduling meetings, supporting committee members' participation, developing agendas with the Chair, and taking minutes.
Plans and participates in presentations and/or exhibits related to health for schools, senior centers, tribal outreach activities, and other community events.
Seeks and participates in training related to Community Health Assessment and Health Improvement for tribal communities.
Assists with ongoing community health assessment activities, including data collection, data review, sharing with the community, and developing recommendations based on assessment results, in collaboration with partners such as AASTEC, IHS, and NMDOH.
Facilitates health and wellness classes, workshops and/or trainings.
Assists with orientation and mentoring of new Health Committee members.
Continuously expands knowledge and understanding of community resources and services to better meet the community health needs.
Participates in local, regional and/or state community health council and planning events/meetings.
Advocates on behalf of the Health Committee
Builds positive relationships with community members, tribal staff and internal and external departments/organizations and strengthens collaboration on improving the health of the Santa Clara community.
Works to reduce cultural and socio-economic barriers between community members and institutions to meet health needs.
Maintains confidentiality of all privileged information.
Contributes to a team effort.
Performs other duties as assigned or required.

Knowledge, Skills, and Abilities
Knowledgeable of community health issues, especially those impacting tribal communities in Northern New Mexico
Ability to interact and communicate effectively with tribal, state, and federal entities and other employees of Santa Clara Pueblo
Ability to educate individuals and small groups and conduct presentations to large groups, including responding to questions and concerns.
Community Health Planner
Ability to organize events, meetings and activities independently and as part of a team.
Proficiency in writing reports and business correspondence
Skill in use of computers and related software applications such as spreadsheets, word processing and presentations
Excellent written and verbal communication skills
Ability to manage multiple tasks and meet deadlines.
Ability to maintain confidentiality of information and records.

Minimum Qualifications
Bachelor's degree in public health, healthcare, social work or a related field plus one-year related experience. Any combination of education from an accredited college or university in a related field and/or direct work experience in this occupation totaling three years may substitute for the required educational experience.
Certifications as a Community Health Worker or Peer Support Specialist can substitute for educational requirement.
Experience time will be considered on an individual basis.
Ability to speak Tewa language preferred, but not required.
Willing to travel to attend training as needed.

Physical Requirements and Work Environment
While performing the duties of this job, the employee is regularly required to sit, use hands to finger, handle, or feel; and talk or hear. The employee frequently is required to reach with hands and arms. The employee is occasionally required to stand, walk, and stoop, kneel, crouch, or crawl. The employee must occasionally lift and/or move up to 25 pounds. If an item is above 25 pounds, lifting shall be done by two or more Tribal staff members. Specific vision abilities required by this job include close vision, color vision, and ability to adjust focus. Work is performed in a professional office environment with a moderate noise level, occasionally working under stressful conditions and for extended periods of time. This position is routinely exposed to Visual/Video Display Terminal (VDT) and extensive personal computer and phone usage. Travel may be required. May be required to work occasional nights and/or weekends. The employee is occasionally exposed to weather.

Employment Requirements
Successful completion of thorough background investigation and MVD check. Employees must comply with Tribal Safety Guidelines, Policies and Procedures and follow the HIPPA US Department of Health and Human Services Guidelines. Successful completion of the mandatory National Incident Management Training (NIMS) within the first 90 days of employment. Must pass a drug/alcohol test. Must have a valid New Mexico Driver's License and insurable through the Tribe's carrier and not have any DUI/DWI convictions within the last five (5) years from any Tribal, State or Federal Court. Must take and pass a current driving course from General Services Administration.
