What are the responsibilities and job description for the LVN Charge Nurse position at Santa Clarita Congregate Living?
Santa Clarita Congregate living is a 6-bed facility with skilled and sub-acute patients, looking for a full time LVN (12 hr. shift) every Tuesday/Wed/Thursday evening shift from 7pm to 7am. The primary purpose of your job position is to provide direct nursing care to the residents, and to supervise the day-to-day nursing activities performed by nursing assistants. Such supervision must be in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility, and as may be required by the Director of Nursing/Clinical Services.
You are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties.
1. Works in office areas as well as throughout nursing service areas.
2. Moves intermittently during working hours.
3. Is subject to frequent interruptions.
4. Works beyond normal working hours on weekends and holidays, and on other shifts/positions when necessary.
5. Is subject to call back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.).
6. Is consistently interacting with residents, families, staff and/or government agencies.
7. Is subject to injury from falls, burns from equipment, odors, etc., throughout the day, as well as to reactions from dust, disinfectants, tobacco smoke, and other air contaminants.
8. Is involved with residents, family members, personnel, visitors, government agencies/personnel, etc.
9. Is subject to hostile and emotionally upset residents, family members, personnel, visitors, etc.
10. Is subject to exposure to infectious waste, diseases, conditions, etc., including TB and the AIDS and Hepatitis B viruses.
11. May be subject to the handling of and exposure to hazardous chemicals.
12. Maintains a liaison with residents, families, and care planning teams to adequately plan for residents’ needs.
Employee must be able to:

2. Speak and write the English language in an understandable manner.
3. Cope with the mental and emotional stress of the position.
4. Have sight/hearing senses, or use prosthetics that will enable these senses to function adequately so that the requirements of the position can be fully met.
5. Function independently and have flexibility, personal integrity, and the ability to work effectively with residents, personnel and support agencies.
6. Push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet.
7. Assist in the evacuation of residents during emergency situations.
