Position: Physical Therapist Hours: 2-6 hours per week

Responsibilities:
Conduct new admission and occasional long-term care evaluations PRN
Provide supervision and e-sign notes for two dedicated Physical Therapy Assistants weekly

Requirements:
Physical Therapy Degree or Doctor of Physical Therapy (DPT) degree from an accredited program
Valid state licensure as a Physical Therapist.
Must be able to complete Physical Therapy Evaluations, Supervision, Collaboration, Safety and Compliance, Education, Plan of Care and Treatment programs, Documentation
Lifting Requirements:
Ability to lift and support up to 50 pounds safely.
Assist patients with mobility, including transfers from bed to wheelchair, etc.
Use proper body mechanics to prevent injury to self and others.
