Position: Physical Therapist Hours: 2-6 hours per week

Location: Santa Fe Care Center Skilled Nursing Facility, Santa Fe, NM 87505 Phone: (505)-982-2574

Responsibilities:

• Conduct new admission and occasional long-term care evaluations PRN

• Provide supervision and e-sign notes for two dedicated Physical Therapy Assistants weekly

Requirements:

• Physical Therapy Degree or Doctor of Physical Therapy (DPT) degree from an accredited program

• Valid state licensure as a Physical Therapist.

• Must be able to complete Physical Therapy Evaluations, Supervision, Collaboration, Safety and Compliance, Education, Plan of Care and Treatment programs, Documentation

• Lifting Requirements:

  • Ability to lift and support up to 50 pounds safely.

  • Assist patients with mobility, including transfers from bed to wheelchair, etc.

  • Use proper body mechanics to prevent injury to self and others.
