Saints Care is funded by the Basic Needs for Postsecondary Students Grant and supports Santa Fe College's (SF) goal to centralize services for students experiencing basic need insecurity and expand support for food security, housing security, transportation security, technology resources, physical and mental health, and career exploration. The primary mission of Saints Care is to assist students in meetings their basic needs so that they can complete their education and gain employment earning a sustainable, living wage.
The Case Manager supports the Saints Care program by fostering student academic achievement through advocacy, empowerment, and removal of barriers to academic completion. The Case Manager provides individualized support for students experiencing basic needs insecurity and is responsible for identifying and recruiting eligible participants; providing basic needs resources; assisting students in academic and career pathway counseling; supporting transfer advising; assisting students with financial literacy training and counseling; and mentoring students in study skills.
- This position is funded by either a grant or another donor-funded source external to the college. Should the funding provided by these special sources be reduced or eliminated, employees will be given as much notice as possible.
Job Description
Responsibilities and Duties Include :
Collaborates with the Coordinator to identify and recruit eligible students to the Saints Care program.Adheres to student eligibility as per the grant and provides individual orientation of program services to students, which includes support services for food, housing, and transportation security as well as support for physical and mental health, technological needs, academic achievement, and career exploration.Supports students by conducting intake assessments to identify their needs, including food, housing, transportation, physical and mental health, technological needs, academic support, and career exploration.Presents the Saint Care program and resources to participants, students, faculty, and other constituents which may include class presentations, college events, and other college initiatives.Provides academic and career pathway counseling to students in the Saints Care program, which includes transfer advising and financial literacy assistance.Assists students with completing transfer applications and financial aid forms and refers them to appropriate SF resources when necessary.Reviews initial Financial Aid Literacy Assessment with students and advises them on how to locate and apply to alternative tuition funding sources, such as scholarships and grants.Advocates for students by helping them navigate complex systems, such as public assistance programs, and connects them to community resources like SNAP, United Way Programs, Housing Agencies, and transportation resources.Connects students with on-campus resources like the SF Food Pantry, the Counseling and Wellness Center, the Learning Commons, and Financial Aid.Builds and maintains a directory of off-campus resources and refers students when needed.Conducts regular check-ins with students to monitor progress and provide ongoing support.Prepares reports detailing outcomes and successes for grant compliance and funding accountability, which includes collecting feedback from students and tracking the utilization and impact of services.Adheres to grant requirements when collecting student data for the evaluation of the programs success and services.Maintains student and grant records in compliance with grant provisions, SF rules and regulations, and federal policies and procedures.Partners with the Coordinator to conduct research necessary for project evaluation and assists in the evaluation process, including the annual report and other related reports.Attends SF trainings and participates in collegewide committees, especially those related to the Saints Care program and Student Life.Works closely with the Saints Care program staff to effectively and holistically serve students.Develops and maintains professional relationships with SF departments and community constituents.Complies with all published College Rules, procedures, guidelines, and laws / regulations governing public employees, including but not limited to those related to document retention and destruction, FERPA, and confidentiality.Strictly honors the privacy, security, and confidentiality of student records and other sensitive information according to appropriate state, federal, and college regulations, policies, and procedures.Provides service excellence through courteous, informed, accessible, and professional engagement.Performs other duties as assigned.Reports to : Coordinator, Saints Care Program
QUALIFICATIONS
Required : A bachelor's degree in education, psychology, social work, or a related field or an associate degree in education, psychology, social work, or a related field with two (2) years of experience as a Basic Needs Case Manager. One (1) to two (2) years of experience in areas of basic needs, student affairs, or similar programs that serve underrepresented groups, preferably low-income, disabled, and / or first-generation students.
Additional Requirements : A criminal background check will be conducted. Applicants must have a valid Florida Class E Operator (driver's) license and be authorized to drive for the college.
Preferred : A master's degree in education, psychology, social work, counseling, or a related field with three (3) years of professional experience; demonstrated success in overcoming barriers like those of the project's target population and / or experience working with this population; and experience in case management, educational advising, recordkeeping, documentation, and project evaluation. Strong communication and technology skills.
