In this role, you will collaborate with patients, families, and healthcare professionals to serve as a vital link as a patient advocate while delivering personalized care plans that promote health, wellness, an enhanced quality of life, and positive outcomes in every aspect of care.

Responsibilities:

• Conduct comprehensive assessments to understand each patient’s unique needs, ensuring personalized and compassionate care.
• Develop and implement individualized care plans in collaboration with physicians, emphasizing dignity and independence.
• Provide essential skilled services, including wound care, medication administration, IV therapy, and catheter care, with a focus on safety and quality.
• Monitor and document vital signs and clinical data using Electronic Medical Records (EMR) to support continuity of care.
• Educate patients and families to empower them in managing chronic conditions and promoting wellness.
• Perform home safety assessments to enhance the living environment and reduce risks.
• Uphold the highest standards of infection control and emergency response practices.
• Act as a patient advocate, fostering trust and building strong, supportive relationships.

Qualifications:

• Proficient in English Communication Skills; bilingual preferred.
• Active RN or LPN license in the state of New Mexico; multi-state preferred.
• Previous experience in home healthcare or a related field is preferred.
• Strong clinical skills in patient assessments, wound care, IV therapy, and chronic disease management and obtaining lab specimens.
• Proficient in Electronic Medical Records (EMR) systems including Wellsky/Kinnser; Authenticate; Training Available.
• Exceptional communication and interpersonal skills.
• Ability to work independently while embodying our mission of compassion and reliability.
• Valid driver’s license, current vehicle insurance, and reliable transportation.
• BLS/CPR
