Responsibilities:
- Conduct comprehensive assessments to understand each patient’s unique needs, ensuring personalized and compassionate care.
- Develop and implement individualized care plans in collaboration with physicians, emphasizing dignity and independence.
- Provide essential skilled services, including wound care, medication administration, IV therapy, and catheter care, with a focus on safety and quality.
- Monitor and document vital signs and clinical data using Electronic Medical Records (EMR) to support continuity of care.
- Educate patients and families to empower them in managing chronic conditions and promoting wellness.
- Perform home safety assessments to enhance the living environment and reduce risks.
- Uphold the highest standards of infection control and emergency response practices.
- Act as a patient advocate, fostering trust and building strong, supportive relationships.
Qualifications:
- Proficient in English Communication Skills; bilingual preferred.
- Active RN or LPN license in the state of New Mexico; multi-state preferred.
- Previous experience in home healthcare or a related field is preferred.
- Strong clinical skills in patient assessments, wound care, IV therapy, and chronic disease management and obtaining lab specimens.
- Proficient in Electronic Medical Records (EMR) systems including Wellsky/Kinnser; Authenticate; Training Available.
- Exceptional communication and interpersonal skills.
- Ability to work independently while embodying our mission of compassion and reliability.
- Valid driver’s license, current vehicle insurance, and reliable transportation.
- BLS/CPR
