Description : POSITION SUMMARY

  • Provides oversight of the assessment and care planning process in the nursing center. Provides general nursing care to residents in retirement and nursing community.

ESSENTIAL DUTIES AND RESPONSIBILITIES

The following duties are normal for this position. This list is not to be construed as exclusive or all inclusive. Other duties may be required and assigned.

  • Interviews residents to determine medical problems / condition and documents in chart for physician.
  • Conducts pre-admission assessments for Residents seeking admission to the nursing center.
  • Evaluates Residents for meeting the criteria for Medicare payment, to include clinical eligibility and availability of Medicare days.
  • Supervises interdisciplinary team in completing a comprehensive Resident Assessment Instrument, to include Minimum Data Set (MDS), Care Area Assessments (CAA) guidelines.
  • Schedules and certifies on-time completion of RAI process for all Medicare assessments.
  • Schedules and certifies on-time completion of RAI process for all residents, regardless of pay source.
  • Monitors daily resident health status to identify significant changes and initiates re-assessment process.
  • Monitors nurse charting to ensure proper compliance for Medicare compliance on each resident as required.
  • Transmits RAI data to Center for Medicare / Medicaid Services (CMS) on regularly scheduled day each week.
  • Communicates with billing personnel to establish Medicare billing accuracy and timeliness.
  • Supervises interdisciplinary team in developing a plan of care for each resident based on assessments.
  • Attend all Resident Care plan Meetings and coordinate scheduling with Social Services Director.
  • Provides education for interdisciplinary team, licensed nurses and unlicensed staff to document services provided according to the plan of care.
  • Conducts regularly scheduled audits of the services provided by internal and external providers for accuracy and timeliness of documentation to validate billing.
  • Explains procedures and treatments to resident to gain cooperation, understanding, and allay apprehension.
  • Rotates among various clinical services of institution as needed.
  • Participates in department or unit quality improvement activities, staff education, infection control program, etc.
  • Maintains compliance to all personnel policies, established community policies and procedures, and Federal and State regulations and standards, including but not limited to HIPAA privacy and confidentiality laws.
  • Participates in an on-call program with other nursing administration.
  • Performs additional duties as assigned by supervisor.
  • May need to respond as appropriate to personal calls from the Independent Living Center. Duties in this setting may include but are not limited to providing First Aid, call emergency personnel (dial 911), and / or call family members at request of Independent Living residents.
  • SUPERVISORY RESPONSIBILITIES

  • Supervise Interdisciplinary Care Team and others as assigned to assure delivery of consistently high quality care to residents and documentation of care per state and federal requirements.
  • Monitor charge nursing staff and certified nurses aides for proper adherence to state and federal guidelines.

    EDUCATION and / or EXPERIENCE

  • Bachelor's degree (B.A.) from four-year college or university; and / or one to three years related experience and / or training; or equivalent combination of education and experience.
  • CLINICAL EXPERTISE

  • Knowledgeable of standards of care and practices applicable to the delivery of geriatric services, complex medical conditions, multiple medical diagnoses, including preventive measures, rehabilitation, and promoting independence.
  • Knowledge, Skills and Abilities :

    Under the following required areas list specific knowledge, skills or abilities necessary to perform this particular job.

  • Language Ability :
  • Ability to communicate effectively in writing and speaking the primary language of the residents.
  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
  • Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization.
  • Mathematical Skills :
  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
  • Ability to calculate figures and amounts such as dosages, fluid intake & output, accurate measurements, proportions, percentages, area, circumference, and volume. Ability to apply concepts of basic algebra and geometry as needed to the delivery of medical services.
  • Cognitive Demands :
  • Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
  • Computer Skills :
  • Ability to use Microsoft Windows. Ability to use software to submit MDS. Ability to use the Internet.
  • Competencies ( Example ) :
  • Ability to operate a car / truck and maintain a valid driver’s license. Ability to operate general office equipment.
  • ENVIRONMENTAL ADAPTABILITY

  • Works primarily indoors in a climate controlled setting
  • Possible exposure to unpleasant odors
  • Possible exposure to chemicals as identified in the MSDS Manual
  • Continuous exposure to residents who are ill, confused, irritable and irrational
