The Medical Records/Admissions Coordinator is responsible for oversight of the admissions process for residents entering our skilled nursing facility. This role includes managing inquiries, guiding families through the admissions process, ensuring proper documentation, and working collaboratively with clinical and administrative teams to facilitate smooth transitions for new residents.
Responsibilities
- Coordinate pre-admission assessments to evaluate potential residents' needs and determine eligibility for admission in collaboration with clinical teams.
- Ensure all necessary documentation, including medical records, insurance information, and legal paperwork, is completed and filed accurately.
- Handle incoming inquiries from potential residents, families, and referral sources, providing accurate and timely information regarding facility services, eligibility, and the admissions process.
- The Admission Coordinator will work closely with the interdisciplinary team, including nursing, social services, and administration, to facilitate admissions and ensure a smooth transition for incoming residents.
- Communicate with hospitals, rehabilitation centers, and other referral sources to obtain relevant information regarding prospective residents.
- Offer resources and guidance to families regarding financial options, including Medicaid and Medicare, as well as private insurance coverage.
- Develop and maintain positive relationships with referral sources such as hospitals, healthcare providers, and case managers to generate admissions leads.
- Maintain accurate records of inquiries, admissions, and resident information in accordance with facility policies and state and federal regulations.
- Ensure compliance with regulatory requirements regarding admissions, including documentation of admission agreements and resident rights.
- Other Duties as Assigned
- High school diploma or equivalent required; Associate's degree in Health Information Technology or a related field preferred.
- Prior experience in medical records or health information management, preferably in a skilled nursing facility or long-term care setting.
- RHIT (Registered Health Information Technician) or similar certification is a plus.
- Proficiency in medical terminology, strong computer skills (especially in EHR software), and a high degree of accuracy and organization.
- Familiarity with HIPAA regulations and best practices for medical record-keeping in healthcare.
- Excellent communication and interpersonal skills, with the ability to build relationships with residents, families, and referral sources.
- Strong organizational and time management skills, with attention to detail.
- Ability to handle sensitive situations with compassion and professionalism.
