Sapphire Nursing at Wappingers located in Dutchess County is seeking an Admissions Intake Coordinator. The Admissions Intake Coordinator is responsible for ensuring a smooth admission transition for the patients entrusted to our care. The primary purpose of this position is to lead and manage the facility's Census, Marketing, and Admissions. You will screen all admission referrals for an appropriate placement, including medical/clinical needs and insurance/financial approvals while maintaining professional relationships with referral sources such as hospital case managers, social workers, and community agencies.
Responsibilities:
- Responds to all referrals in system in a timely manner (within 0-10 mins).
- Manages Admissions office in absence of the Director.
- Completes Admissions agreements within 24-48 hours of the patient’s admission.
- Coordinates and facilitates appropriate admission to our facility.
- Maintain contact with patients/families regarding potential admissions.
- Conduct interviews and tours of the facility.
- Verify Insurance & SOR, obtain authorizations for Admissions.
- Educates family members, clients and prospective accounts regarding services and programs offered by our facility
- Generates new referrals by establishing relationships social workers, case managers and other appropriate professional contacts within hospital accounts or other assigned accounts
- Maintains current knowledge and understanding of client payment sources, i.e., HMO, commercial insurance, Medicare, Medicaid, etc.
- Contacts family members and social workers when referral is received and accepted.
- Welcomes patients on admission.
- Notifies departments of anticipated admissions.
- Conducts audit of empty rooms, making sure they are cleaned and all equipment in the room is in working order.
- Adheres to all policies and procedures of the Nursing home.
- Performs other duties as assigned.
Qualifications:
- Minimum Associates Degree preferred.
- Minimum 2-5 years experience as a successful Admissions Coordinator in a short term rehabilitation/long term care facility preferred
- Healthcare, sales, marketing experience preferred.
- Knowledge of Microsoft Office, medical terminology .
- Excellent typing and data entry skills.
- Excellent interpersonal skills; possess the ability to deal tactfully with residents, family members, visitors.
