Position Summary
Performs various personal care services relative to the needs and comfort of residents and assists with related nonprofessional services that contribute to Quality of Life. Performs restorative services as determined by restorative nurse/therapist to maintain each resident’s highest level of functioning.
Essential Duties & Responsibilities
- Check vital signs, weights, and answers call bells promptly.
- Every 2 hours, toilet residents, checking for incontinence. Documents vital signs timely, obtains accurate weights and reweighs if a discrepancy. Completes daily skin assessments (weekly with Charge Nurse).
- Communicating, documenting any mental or physical changes on residents. General assessments should be done at every opportunity, including showers.
- Always ensures resident safety, including checking alarms and call bells, monitoring and adhering to footrest policy, etc. Knowledge of Falling and Falling Star program.
- Feeding residents and monitoring intake, providing snacks, fresh drinking water, and other nourishments. Knows resident diets and feeding strategies.
- Performs resident hygiene, grooming, showering, nail care.
- Assists residents with mobility, range of motion exercises, transferring, transporting.
- Accurate and detailed documentation of all care provided to residents including intake and output if needed.
- May work or assist in Personal Care department.
Non-Essential Duties & Responsibilities
- Encourages diversional activities to confused or combative residents. Provides support and / or companionship.
- Prepare and assist residents for therapy appointments, activities, doctor’s appointments, family outings, etc.
- Secure and stock any resident supplies needed with cost considerations.
- Attends mandatory in-services and monthly nurse meetings.
- Performs other duties and responsibilities as directed.
Qualifications
Qualifications
- High school education.
- Must maintain current Nurse Assistant Certificate and a minimum of 12 hours of annual training.
- Effective communication and organizational skills.
Physical & Visual Requirements
- Stands, walks, listens, uses hands, pushes, and pulls, reaches most of the day.
- Lifts up to 25 lbs most of the day, up to 50 lbs and sits, stoops, kneels, & crouches 1/3 to ½ of the day.
- Keen sense of vision (with or without correction) necessary for close inspection and attention to minute details and small objects, and hearing to hear residents, alarms, and instructions.
- Ability to detect and react quickly to slight motions and/or the ability to distinguish colors accurately.
- May be exposed to infection, odors, and psycho-behavioral problems.
Salary : $18 - $22
