Position Summary
Provides nursing care to meet the needs of Personal Care residents in Zurn Pavilion, Reed Manor, and Reed Terrace.
Essential Duties & Responsibilities
- Monitors Med Techs and all PCAs under your charge to ensure quality resident care. Maintain positive attitude and sets a professional example. This includes making several rounds per shift to ensure proper care is being provided, talking with staff on resident improvement or decline. This also includes fully staffing, redirecting and coverages for all 4 units, monitoring staff administration of medications and charting of medications, oral, enteral, intramuscular, or intravenous on respective shift and ensuring resident care plans are followed. Monitors that all available employees answer call bells and conducts call-bell audits. Counsels and disciplines staff appropriately and re-trains and mentors to correct any inappropriate behaviors or procedures.
- Reviews resident’s condition changes. Ensures physicians, families and administration are notified whenever necessary. Considers the interest of SAR in family complaints or emergency situations. Informs Personal Care Home Administrator and PC Nurse Manager of all incidents requiring transfer to the hospital.
- Makes rounds with physicians and ensures orders are obtained as needed and in conformance with established standards.
- Performs venipuncture and IV therapy as ordered by physician. Transcribes Physician orders when necessary and ensures that two staff members have noted them for accuracy.
- Obtains report and gives verbal report to the oncoming RN Supervisor, LPN, or Med Tech. Passes on information needed for follow-up and signs the 24-hour report form.
- Secures prescribed medications; performs direct services to residents such as administering medications and treatments, if deemed necessary. Dispose of medications per facility policy.
- Review medication records for completeness of information, accuracy in transcribing of Physician orders and adherence to stop orders.
- Recognizes and reports resident problems to Personal Care Home Administrator and PC Nurse Manager. Communicates effectively with all staff.
- Follows up on recommendation by the Dietician. Ensures weekly/monthly weights are done and accurate. If a 2 lb. weight loss or more resident must be re-weighed.
- Attends in-services and any other related duties and responsibilities that may become necessary as directed by Personal Care Home Administrator and PC Nurse Manager.
- Monitors dining room and completes dining room QA at least monthly. Ensures the staff is knowledgeable of diets and feeding programs.
- When Admission Nurse is not present- Ensures Admission and Transfer process is followed and documented. Corrects medical record audit. Obtains Physician orders for all new admissions/readmissions. Coordinates a smooth admission process with staff and residents. Assures Assessments are completed; body audits are done within 2 hours of admission and signed by two licensed nurses. All shift supervisors must review admission charts to ensure all assessments are completed correctly and signed.
- Upon discharge, plans and initiates resident and family health teaching and rehabilitation.
- Ensures Incident Reports are documented in the computer and follow-up has been done, i.e., family notification, physician notification; and proper intervention is initiated. The DON or designee must be notified of all incidents requiring a transfer to the hospital immediately after the incident has occurred. Ensures interventions are in place to prevent reoccurrence.

Non-Essential Duties & Responsibilities
- Provides individual resident care as needed such as bathing, feeding, ambulating, and/or toileting.
- Prepares medication cart for the next shift

- Other duties as assigned by Personal Care Home Administrator and PC Nurse Manager.
Qualifications
Qualifications
- Gerontology experience preferred.
- Graduate of an approved school of nursing and registered in the Commonwealth of Pennsylvania.
- Continuing education as required to retain license and stay abreast of changes in the profession.
Knowledge, Skills, & Abilities
- Knowledge and interest in geriatric and restorative nursing care.
- Knowledge of governmental laws for Nursing Practice, including Abuse Policies and Procedures.
- Recognizes and reports safety hazards and knows emergency procedures.
- Effective communication and organizational skills.
Physical & Visual Requirements
- Frequent lifting up to 40 lbs, standing, walking, bending, squatting, kneeling, reaching, grasping, fine manipulation, pushing, and pulling.
- Occasional sitting, twisting, and lifting over 50 lbs.
- Keen sense of vision (with or without correction) necessary for close inspection and attention to minute details and small objects. Ability to detect and react quickly to light motions and/or the ability to distinguish colors accurately.
- Must be able to hear alarms on equipment, client calls, and instructions from physician/departmental staff.
- May be exposed to infection, odors, and behavioral management situations.
