LPN-Full job description
Qualifications:
Graduate of an accredited, state licensed School of Nursing; Current
Licensed Practical Nurse license; Works well with seniors in providing nursing care.
Responsibilities
- Administer emergency CPR/First Aid as needed.
- Administer medications as ordered by the participant’s physician and under RN supervision.
- Dispose of expired/discontinued medications appropriately.
- Provide skilled nursing duties under RN supervision.
- Document any necessary medical observations and daily record keeping.
- Update participant medication orders, treatments, and medical questionnaires as required.
- Notification of participant responsible party and physicians as required.
- Complete Behavioral and Administrative Reports as needed.
- Monitor participant vital signs as required.
- Participate in assessment and Person-Centered Plan of Care conferences.
- Conduct staff in-service/education on relevant topics as required.
- Assists participants with activities of daily living, personal care, ancillary services and in the participation of activities as needed.
- Ability to safely lift and transfer.
- Request supplies and items needed for nursing through the Program Director.
- Responsible for environmental safety and sanitation.
- Follow Standard Precautions at all times.
- Comply with all HIPAA and confidentiality requirements.
- Participate in staff meetings, in-service training, group discussions, and other relevant training.
- All other jobs and duties as required.

Qualifications:
High school diploma or equivalent; CPR certified and First aid, Minimum two years’ experience in a supervised position providing personal care and/or activities preferred; Outgoing personality, Works well with seniors in providing recreational activities.
Responsibilities
- Assist participants with transferring, ambulation, and personal care, including bathing, grooming, and toileting as needed.
- Monitor and assist participants with nutritional intake, as needed.
- Administer emergency CPR/First Aid as needed.
- Participate in participant Person-Centered Plan of Care as needed and adhere to planned intervention(s).
- Plan and coordinate activities under the supervision of the Activity Coordinator.
- Assist participants in the participation of activities.
- Assist participants to various activities or events.
- Request supplies and items needed for projects through the Activity Coordinator.
- Responsible for participant safety and environmental sanitation.
- Follow Standard Precautions at all times.
- Comply with all HIPAA and confidentiality requirements.
- Participate in staff meetings, in-service training, group discussions, and other relevant training.
- Bilingual-English and Vietnamese
