This role requires a commitment to safety, customer service, and the wellbeing of our residents. May assist Program Director, Life Enrichment, with other in-house activities. This is a Tuesday-Saturday work week.

POSITION REQUIREMENTS / QUALIFICATIONS:
- Must be at least 18 years of age.
- Must pass State and Company criminal background/drug screens.
- Current, valid State driver’s license.
- Minimum of five (5) years of relevant driving experience as a licensed driver, with good driving record.
- Demonstrates effective time management skills.

ESSENTIAL DUTIES:
- Drives vehicle (car or van) to destination designated by the facility leadership.
- Inspects company vehicle for defects before and after trips using company vehicle checklist and reports any discrepancies indicating condition to supervisor.
- Maneuvers vehicle in and around tight corners, through business areas and around high pedestrian traffic.
- Assists residents in and out of vehicle recognizing each resident's conditions and restrictions.
- Follows the standards, policies, and procedures of Five Star and demonstrates good judgment when transporting residents to appointments.
- Makes appropriate observations while transporting residents and reports the information to the appropriate professional within the Community.
- Observes all state traffic laws and any other state work assignments required by the driver.
- Conducts self in a positive, professional manner in compliance with company policies and procedures.
- Transports residents to all appointments or activities in a safe and timely manner.
- Adjusts work schedule to facilitate activities/duties as needed.
- Complies with necessary documentation requirements while transporting a resident or for Company business.
- Organizes groups of residents for each scheduled activity.
- Picks up and delivers residents to routed stops.
- Keeps vehicle clean and in working condition.
- May assist Program Director, Life Enrichment, with in-house activities including planning and organizing, arranging furniture, decorating and general set up for activities.
- Performs other duties as assigned.

- Attends in-services and educational programs and attends continuing education as required.
- Respond to the needs of residents, families and visitors by providing immediate assistance.
- Function as a member of the team, assisting and supporting co-workers as needed.
- Use personal protective equipment when at risk of exposure to blood or other potentially hazardous body fluids and all other times indicated by facility policies and procedures.
- Reports any signs of rodent or pest infestation to the supervisor immediately.
- Familiarize self with Material Safety Data Sheets (MSDS) and Universal Worker Precautions for all housekeeping chemicals and cleaning supplies.
- Reports injury to self and others to supervisor immediately.
- Participants in the facility’s Quality Improvement program, as directed.
- Comply with all documentation policies/procedures.
- Use proper body mechanics when bending or lifting.
- Maintain knowledge of and comply with the community’s Infection Prevention policies and procedures.
- Know the community’s disaster and fire safety policies and procedures, including the location and correct use of the fire alarm system.
- Perform all duties according to policy in a professional manner.
- Comply with community attendance policies.
- Comply with community dress code policies.
- Attend scheduled staff meetings and/or in-service programs.
PHYSICAL DEMANDS:
- Frequently sit, stand and/or walk for extended periods of time, stand, carry, finger, grasp and feel.
- Lift, push or pull a minimum of 50 pounds.
- Ability to safely transfer residents between vehicle, vehicle seats and wheelchairs.
- Ability to secure residents and/or their wheelchairs or assistive devices.
- Ability to safely operate any applicable wheelchair lift.
- Perform tasks that require bending, reaching, and manual dexterity.
- Vision abilities required by this job includes distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
- Must be in good general health and demonstrate emotional stability.
- Must be able to cope with the mental and emotional stress of the position which includes adherence to strict time constraints.
- Occasionally required to lift and/or move items weighing more than 50 pounds.
EXPOSURE RISKS:
- Is subject to potential exposure to infectious waste, disease, conditions, etc., including but not limited to, TB, HIV and Hepatitis B.
- May be exposed to varying weather conditions.
- May be exposed to vehicle–related fumes.
