About Us: Save the Michaels of the World is dedicated to providing comprehensive support and services to individuals experiencing homelessness, particularly those with mental health and substance use disorder challenges. Position Overview: We are seeking 2 dedicated and empathetic Care Managers to join our team. The ideal candidate will have a strong background in social services, mental health, and addiction support. This role involves working closely with unsheltered individuals, people coming out of inpatient treatment and incarceration in a busy drop-in center environment to develop and implement personalized care plans, ensuring they receive the highest level of service and support.
Key Responsibilities:
- Conduct comprehensive assessments of clients’ needs, strengths, and goals.
- Develop and implement individualized care plans in collaboration with clients and other service providers.
- Provide ongoing case management and support to clients, including crisis intervention and advocacy.
- Coordinate access to mental health, addiction treatment, housing, and other essential services.
- Build and maintain strong relationships with community partners and service providers.
- Monitor and evaluate clients’ progress, adjusting care plans as needed.
- Maintain accurate and up-to-date client records and documentation.
- Participate in team meetings, training sessions, and professional development opportunities.
Qualifications:
- Associates degree in Social Work, Psychology,Counseling, or a related field
- Minimum of 2 years of experience in case management, social services, or a related field.
- Strong understanding of mental health and addiction issues, particularly within the homeless population.
- Excellent communication, organizational, and problem-solving skills.
- Ability to work independently and as part of a multidisciplinary team.
- Compassionate, non-judgmental, and committed to advocating for clients’ needs.
- Positive attitude towards coworkers and the organization
- Valid driver’s license and reliable transportation.
Job Type: Full-time
Pay: $42,000.00 - $45,000.00 per year
