The Job

The Health Promotion Representative organizes and delivers health, technology, social, and exercise classes to older adults and family caregivers through various platforms, including in-person, telephone, and video conferencing. This role involves collaborating with a multidisciplinary team to develop and implement comprehensive educational programs that support the health and independence of older adults and caregivers. The individual conducts assessments with local community agencies, designs, implements, presents, and evaluates health programing to ensure they meet the specific needs of low-income senior communities. They also build and maintain strong relationships with community-based partners to improve access to health education information and promote Independence at Home’s (IAH) older adult supportive services. Additionally, they review, develop, and recommend high-quality, culturally appropriate topic specific health education materials, maintain accurate records, and document program outcomes to support continuous quality improvement initiatives to improve the effectiveness of partnerships and health education programs.

You Will

Identify, cultivate, and sustain strategic partnerships with various organizations, including nonprofit entities, government agencies, and senior centers, while engaging in no-cost partnership agreements and ensuring the educational mission is clearly communicated.

Act as the primary liaison between the organization and partners, scheduling regular check-ins and collaborating on outreach efforts. Develop and cultivate relationships with community agencies, senior apartment buildings, and senior-serving sites to increase class scheduling and enhance visibility of IAH support programs, while assisting in the identification and facilitation of health education and outreach presentations in community settings.

Lead Trading Ages training for older adult senior-serving professionals, including case managers, social workers, nurses, senior center staff, government workers, and others who interact with older adults. This interactive age sensitivity training provides participants with first-hand experience of the challenges associated with aging. The role requires strong instructional and message delivery skills to effectively engage with professionals, including the ability to adapt teaching strategies to diverse learning styles, facilitate interactive and inclusive sessions, and communicate complex information clearly and concisely.

Build and maintain strong relationships with community partners to schedule in-person, telephonic, and online health and wellness classes. Leverage partnerships to boost community engagement and raise awareness of IAH’s services.

Schedule and manage health education classes with community partners for outreach and marketing. Use strong instructional skills to teach diverse adult audiences, including older adults, caregivers, and professionals, adapting strategies for varying literacy levels. Deliver classes across in-person, virtual, telephonic, and hybrid formats.

Implement marketing strategies across social and print media to promote health education programs, creating culturally relevant materials like flyers, e-newsletters, and social media posts to boost older adult class participation. Engage in ongoing activities that build community partnerships, representing IAH at community events, social service networking functions and industry forums to expand awareness, strengthen community ties, and advocate for senior health education.

Represent IAH at community events, senior health fairs, and forums to promote senior health education and seek new partnership opportunities, highlighting IAH’s positive impact on older adults.

Implement, administer, and teach IAH-developed classes and evidence-based programs for older adults, including health education, exercise, and fall prevention. Develop or adapt educational materials to meet community needs and stay current on best practices through ongoing training in chronic disease management, cognitive health, and other relevant topics.

Evaluates and updates the curriculum in collaboration with the Manager and Health Education team, while tracking key performance indicators such as program attendance and participant engagement. Gathers, enters, and compiles data to assess the impact of IAH services, reviews feedback reports to improve partnerships, and ensures ongoing support and feedback collection from partners.

Adhere to all quality, compliance and regulatory standards to achieve IAH and SCAN outcomes.

Your Qualifications

Associate's Degree or equivalent experience.

Bachelor's Degree or equivalent experience in Gerontology, Healthcare Administration, Public Health, Social Work, or related field, preferred.

BILINGUAL- Must be bilingual in English / Spanish. (Test will be administered to assess proficiency.)

Travel 50 % of the time in the assigned area including, but not limited to : Alhambra, Glendora, Azusa, Temple City, San Marino, Pasadena, San Dimas, Montebello

Must have a valid driver’s license, automobile insurance and reliable transportation.

Certified Health Education Specialist (CHES), preferred but not required.

2-4 years of experience in community outreach or nonprofit programs involving older adults and / or caregivers. Experience teaching, coaching, or tutoring adult learners. Strong project management skills to coordinate and present at IAH educational events in partner locations, ensuring proper scheduling, setup, and material presentation.

Laptop Computer and LCD Projector Usage : Must be comfortable using a laptop computer and an LCD projector. This includes the ability to navigate operating systems, run necessary software, and set up and operate projection equipment for presentations and meetings.

Ability to comply with record-keeping protocols. Must maintain accurate and detailed records, ensure data is entered correctly and consistently, and follow all organizational guidelines and procedures for data management.

Ability to work collaboratively : Must be able to effectively work in a team environment, fostering open communication and active listening.

Must manage multiple tasks, stay self-motivated, deliver quality results, and handle multiple deadlines. Adaptability : Openness to learning and adopting new technologies and teaching techniques.

Ability to creatively deliver training materials in an in-person and online setting and have strong presentation skills to effectively address diverse audiences.
