The Job
Responsible for implementing Population Health Management (PHM) strategy to execution in collaboration with internal business/IT stakeholders within the Health Plan and with external stakeholders and provider groups. Define and consolidate PH strategy, facilitate cross-functional discussions based on Population Health and NCQA guidelines/standards. Facilitate cross-functional teams to share insights, design solutions and implement solutions and processes.
You Will
Defines and leads implementation of PHM strategy rooted in data. Leads PHM-dedicated team to consolidate and synthesize cross-functional inputs to build cohesive strategic plan.
Responsible for leading day to day strategy to execution for all PHM programs and services in collaboration with internal business/IT stakeholders and external provider groups. This includes program update, program changes, risks and issues associated to the program and escalating to Senior leadership for action. The Population Health portfolio includes initiatives related to Behavioral Health, Health Equity, Chronic Disease Management and Health Education.
Performs root cause analysis for PHM opportunities using relevant techniques and data. This role facilitates strategy and design in collaboration with cross-functional teams to design and implement solutions and processes.
Understands and applies PHM and NCQA guidelines/standards for the identification of business needs, data needs and provider needs to identify opportunities and develop solutions and processes.
Synthesizes information and partners with internal and external teams to create business and functional specifications/requirements by facilitating cross functional meetings and workshops of business and IT stakeholders.
Collaborates with Senior Leadership to align on PHM interventions and company direction to inform cross-functional decision making.
Analyzes data, translates insights from the data, coordinates data collection and analysis to measure impact of designed interventions based on defined key performance indicators and PHM outcome metrics.
Partners with the project management to create an action plan for implementing recommendations. Tracks and develops mitigation plans (where possible) for risks and constraints facing the team.
Fosters positive working relationships with internal and external stakeholders to gain buy-in and enable seamless implementation.
Communicates effectively with stakeholders at all levels to identify needs and evaluate alternative business solutions with support from project management.
Continually seeks opportunities to increase customer satisfaction and deepen client and vendor relationships.
Functions as an expert in process redesign and process expert to the team, providing guidance as needed. Develops talent in PHM team and across the organization to build institutional awareness of PHM interventions.
Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies.
Actively supports the achievement of SCAN’s Vision and Goals.

Your Qualifications
Bachelor’s degree in a relevant subject area such as Public Health, Nursing, Health Administration, Social Work, Business Administration, or related field. MBA is preferred. A comparable combination of education/experience and/or training will be considered equivalent to the education listed.
Proficient in applications (i.e. Visio and Workflow Diagrams) and experience to effectively communicate current state and future state, including program evaluation.
8 years of professional experience.
8 years of healthcare experience.
Relevant experience in the management of population health related programs.
Strong understanding of Project Management discipline.
Excellent presentation skills with ability to communicate a visual story.
Ability to communicate difficult/sensitive information tactfully.
Ability to facilitate effective cross functional team interaction.
Strong experience in writing business and functional requirements and facilitating workshop sessions to gather requirements.
