Job Overview
We are seeking a dedicated and compassionate Service Coordinator to join our team. The ideal candidate will play a crucial role in supporting children ages 0-3 and their families by coordinating services and ensuring they receive the appropriate care and resources. This position requires strong interpersonal skills, a commitment to client welfare, and the ability to work collaboratively with various stakeholders in the developmental disability field.
Duties
- Act as the single point of contact for families enrolled in Ohio Early Intervention.
- Coordinate initial and ongoing eligibility and Individualized Family Service Plan (IFSP) meetings.
- Assist families with identifying, obtaining, funding and monitoring needed Early Intervention services.
- Assist with developing outcomes that reflect the families concerns and priorities.
- Assist families with locating and connecting to other supports and resources that are wanted and needed.
- Facilitate the development of a transition plan before the child’s third birthday.
- Collaborate with the entire Early Intervention team, supporting and building upon each other’s strengths.
Experience
- Bachelor’s or Graduate’s degree from an accredited college or university in one of the following fields of study: Audiology, child and family studies, child development, child life, counseling, education, hearing and speech sciences, human development, human ecology, human social services, medicine, nursing, nutrition science, occupational therapy, ophthalmology, optometry, physician’s assistant, physical therapy, psychology, rehabilitation, social work, or speech/language pathology OR
- Bachelor’s or Graduate’s degree from an accredited college or university in any field and has at least 3 years of verified full-time supervised, paid experience in supervision, case management, early intervention service coordination, and or working with children birth through age five with disabilities, developmental delays or diagnosed physical or mental conditions.
- Must have or be eligible for Ohio DODD Certification.
- Demonstrate ability to foster and maintain an A attitude within all areas of external/internal interaction.

Pay: From $21.00 per hour
