About the Role
- Maintain and/or improve residents' ability to control everyday physical needs and mental/psychosocial needs.
- Identify individual social and emotional needs.
- Develop and maintain individualized care plans.
- Participate in the planning of resident admissions, community re-entry or transfer to another facility.
- Participate in interdisciplinary care plan meetings to ensure treatment of social and emotional needs of residents.
Requirements
- Bachelor's Degree in Human Services; Psychology or Social Work required.
- Long Term Care/Subacute experience necessary.
