Job Summary:

The Case Manager II plays a pivotal role in coordinating patient care through effective collaboration with the Interdisciplinary Care Transitions team. They ensure optimal resource utilization, service delivery, and compliance with external review agencies while providing ongoing support and expertise throughout the care continuum.

Key Responsibilities:

Care Coordination:

• Collaborates with the Interdisciplinary Team and Physicians to coordinate clinical and psycho-social activities.
• Maintains up-to-date knowledge on reimbursement modalities, community resources, case management, psychosocial, and legal issues affecting patients and providers.
• Identifies high-risk patients requiring additional support and refers them appropriately.
• Serves as a patient advocate, enhancing collaborative relationships to inform decision-making.
• Demonstrates age-appropriate care knowledge for diverse patient populations.
• Promotes interdisciplinary collaboration to achieve treatment goals and optimize resource utilization.
• Cultivates strong relationships with clinical staff, physicians, patients, families, post-acute providers, payors, and other stakeholders to plan comprehensive patient care.

Discharge Planning:

• Conducts ongoing assessments to provide timely and safe discharge planning.
• Develops and executes effective discharge plans, utilizing critical thinking and collaboration with patients/families.

Utilization Management:

• Reviews medical necessity for appropriate service utilization from admission through discharge.
• Promotes efficient utilization of clinical resources.
• Coordinates continued stay authorization with payors through timely and accurate clinical reviews.
