Key Responsibilities:
Care Coordination:
- Collaborates with the Interdisciplinary Team and Physicians to coordinate clinical and psycho-social activities.
- Maintains up-to-date knowledge on reimbursement modalities, community resources, case management, psychosocial, and legal issues affecting patients and providers.
- Identifies high-risk patients requiring additional support and refers them appropriately.
- Serves as a patient advocate, enhancing collaborative relationships to inform decision-making.
- Demonstrates age-appropriate care knowledge for diverse patient populations.
- Promotes interdisciplinary collaboration to achieve treatment goals and optimize resource utilization.
- Cultivates strong relationships with clinical staff, physicians, patients, families, post-acute providers, payors, and other stakeholders to plan comprehensive patient care.
- Conducts ongoing assessments to provide timely and safe discharge planning.
- Develops and executes effective discharge plans, utilizing critical thinking and collaboration with patients/families.
- Reviews medical necessity for appropriate service utilization from admission through discharge.
- Promotes efficient utilization of clinical resources.
- Coordinates continued stay authorization with payors through timely and accurate clinical reviews.
