Job Summary
We are seeking a highly skilled Registered Nurse Care Manager to join our team at ScionHealth. This role involves providing exceptional patient care by utilizing nursing processes such as assessment, planning, intervention, and evaluation.
Key Responsibilities
- Assesses patient needs and develops comprehensive care plans in coordination with interdisciplinary teams.
- Provides direct and indirect patient care, including administering medications and performing treatments.
- Maintains accurate and complete documentation of patient assessments, care plans, and other pertinent information.
- Collaborates with physicians and other healthcare professionals to ensure seamless patient care.
Requirements
- Degree from an accredited Bachelor of Science in Nursing or Associate Degree in Nursing program.
- Current state licensure as Registered Nurse.
- BCLS certification required; ACLS certifications preferred.
- Minimum six months' Medical/Surgical experience in an acute care setting preferred.
