About the Role:
As a Wound Care and Ostomy Advanced Practice Nurse, you will be responsible for providing direct patient care, developing and implementing individualized care plans, and collaborating with interdisciplinary teams to achieve optimal patient outcomes.
Responsibilities:
- Provide quality patient care through the nursing process of assessment, planning, intervention, and evaluation.
- Develop and implement individualized care plans in collaboration with other healthcare professionals.
- Maintain accurate records and documentation of patient assessments, care plans, and treatment progress.
- Collaborate with physicians and other healthcare professionals to coordinate patient care and treatment.
- Participate in discharge planning and educate patients and families on post-discharge care.
Requirements:
- Current state licensure as a Registered Nurse.
- BCLS certification and ACLS required.
- Minimum one year training and experience in Critical Care.
