Job Description
- Develops and implements individualized patient care plans based on assessment findings and medical history.
- Provides direct patient care, including administering medications, performing treatments, and educating patients and families.
- Maintains accurate and complete documentation of patient assessments, care plans, and other pertinent information.
- Collaborates with interdisciplinary teams to ensure seamless patient care and effective communication.
