Patient Advocacy and Discharge Planning
In this role, you will serve as a patient advocate, enhancing collaborative relationships to maximize patient decision-making. You will also conduct comprehensive discharge planning for each patient, utilizing critical thinking to develop and execute effective plans. Additionally, you will coordinate and communicate with patients/families efficiently and effectively.
You will conduct medical necessity reviews for appropriate service utilization from admission through discharge. This role promotes effective and efficient utilization of clinical resources, conducting timely and accurate clinical reviews, care collaboration, and continued stay authorization with payors.

Qualifications:
- Graduate of an accredited program required for RN; BSN preferred; or MSW/BSW with licensure as required by state regulations.
- Healthcare professional licensure required as Registered Nurse, or Licensed Clinical Social Worker (LCSW) or Licensed Social Worker (LSW) if required by state regulations.
- Certification in Case Management a plus.
