What are the responsibilities and job description for the SOCIAL WORKER position at SCO Family of Services?
Our Children’s Community Residences, located in Queens and Long Island, provide a structured, supportive, home-like environment for adolescents who are ready for discharge from higher levels of care such as Residential Treatment Facilities and psychiatric hospitals, but are not yet ready to return home.
While in our program, working in conjunction with their family supports, youth learn the skills necessary to live and function as active members of the community, while learning to adopt the necessary coping, self-care and independent living skills necessary to successfully integrate back to the family home and community.
I. SPECIFIC RESPONSIBILITIES
- Coordinate and schedule screenings, intakes and admissions of potential residents for placement in Crisis Respite and Community residence programs (CR & CCR).
- Facilitate and schedule Service Plan reviews (initial, quarterly, annual) with multi-disciplinary team of staff, service providers, child and family in the development/update of goals
- Administer CANS assessment to all residents at allocated intervals.
- Completion of Biopsychosocial Assessment at time of admission and update annually.
- Participate/facilitate parent training groups/meetings and family engagement program activities monthly.
- Arrange/Coordinate sibling and family visits/contacts in the program, community and home
- Provide crisis de-escalation with children and meet with them individually and in groups to work on specific service plan/ treatment goals.
- Participate in bi-weekly SPOA Out-of-Home Placement Committee meetings as assigned
- Work in conjunction with the CR Team and Family around the development of aftercare and disposition/ discharge planning
- Schedule and coordinate weekly force fields with input from staff and parents
- Provide clinical interventions and training to child and family around issues that led to child’s placement as well as for goals established
- Assist with transportation of residents to Dr appts., school, recreational activities, and coverage as needed
- Maintain contact with service providers, including day treatment, hospitals, schools and outpatient clinics.
- Maintain case records and required documentation for both CR and CCR programs
II. QUALIFICATIONS
- MSW-(LMSW preferred) with 2 years of experience working with children with behavioral and mental health challenges (SED).
- Valid New York State Driver’s License
- Experience working with children and families
- Certification in Child Adolescent Needs and Strengths-NY (CANS-NY); Can certify upon hire
- Acceptable clearance checks for State Central Register, Driver license and fingerprinting as required by the program.
- To attend Non-Violent Crisis Intervention training for certification/re-certification (requirement during probationary period).
