- Under the direction of the Senior Director, Population Health, the Transition of Care (TOC) Navigator- lead serves as the initial point of contact for referring physicians, patients and caregivers to provide resources and assistance with accessing clinical and supportive care services. North Carolina or Multi-State RN licensure is required for this position.
