- Primary contact / advocate for patients in a large multidisciplinary gastrointestinal oncology practice. \n
- Collaborate with care team to develop and facilitate individualized plan of care. \n
- Educate patient and caregivers about disease, treatment modalities, side effect management and self-care measures. \n
- Coordinate all aspects of care including diagnostics, procedures, referrals, treatments. \n
- Triage all communications including patient calls and emails, other departments, insurances, genomic testing organizations. \n
- Formed strong relationships with all colleagues including physicians, nurses, clerical staff, and management across multiple departments and locations. \n
- Navigating patients through the continuum of care acting as a liaison and resource between the physician, other departments, and the patient. \n
- Establish specific, measurable, attainable, relevant and time specific nursing goals to guide the care planning process and promote self-reliance. \n
- Provide in person, clinic visits as needed at the initiation of care and throughout transitions of care throughout all of our hematology / oncology clinic sites, as needed. \n
- Formulating custom care plans from assessment and clinical information demonstrating the application of clinical guidelines including standards of care. \n
- Providing disease and treatment specific education tailored to meet patient's individual goals of care. \n
- Onsite rounding throughout treatment plan implementation as needed to provide needed in person patient evaluation. \n
- Provide direct, hands-on nursing care, as needed. \n
- Assist patients in establishing, modifying, or reinforcing a social support network, thereby promoting self-reliance. Assisting patients with social services needs by identifying resources in the community and referring to these programs. \n
- Collaboration with system wide authorizations team, scheduling in other departments, as needed to ensure patient receives timely and comprehensive care. \n\n\n\n#LI-KL1\n\n\n\n\n\n\n\n\n\nRequired Education / Experience / Specialized Skills : \n
- BSN plus 3 years recent clinical nursing experience in a variety of clinical settings. \nRequired Certification / Registration : \n
- CA RN License \n
- BLS if working in a clinical setting. \n
- ONS Chemotherapy and Biotherapy. \nPreferred Education / Experience / Specialized Skills / Certification : \n
- MSN \n
- OCN if working in Hematology ONN-CG if working in Oncology area. \n
