Salary Range: $77,968 - $80,000

Position Summary:

Seeking a Full-time Care Manager for our Care Management department in Aberdeen, WA. The Care Manager supervises an interdisciplinary care management team assisting high-risk patients with behavioral health and medical concerns to meet complex needs and to achieve better health outcomes. The Care Management team consists of the Care Manager, Integration Specialists, and Care Management RNs. This team provides Health Home contracted services as well as Care Management services for other Sea Mar patients. 

The Care Manager is responsible for assigning to the team tasks of timely and effective screening and assessment, health action care planning, and referrals to and communication with both internal service providers and community-based resources. Screenings may pertain to functional abilities, daily self-management skills, level of activation, depression, anxiety, drug and alcohol use, and other screenings when indicated (examples may include PHQ-9, GAD-7, PAM/CAM, KATZ ADL, AUDIT, DAST, Pain, Fall Risk, etc.).

The Care Manager will also assign team members to provide services which are aligned with the Health Home six core services; Intensive Care Coordination, Health Promotion, Care Transitions, Referral to Community Resources, Individual and Family Support. Care Management team members will create a health action plan setting self-management goals with the patient and will help the patient increase his/her level of activation to meet these goals.

Requirements:

• Understanding of health concerns that compound self-care, and an understanding of chronic medical conditions.
• Working knowledge of chronic disease management interventions and evidence-based chronic care guidelines.
• Ability to supervise and train new or current integration specialists, care coordinators, case managers and other staff regarding Care Management duties and scope of work.
• Ability to educate staff on the medical and psychosocial needs of each patient served.

Education and/or Experience:

• MSW, MA, MS in counseling or similar human service field or RN with social service experience is preferred.
• Bachelor’s level education with three years of care coordination or case management experience and supervisory experience will be considered.
• Experience working with underserved, transient populations.
• Experience working with individuals with challenges related to social determinants of health.
• Experience working with individuals with chronic conditions, substance use disorders, chronic mental illness, and crisis intervention.
• Experience supervising employees in a health or social service organization environment.
• Bilingual Spanish/English preferred.
