Full-Time: 32 hrs/wk, Monday-Friday from 3:00pm-11:30pm with rotating weekends
DUTIES AND RESPONSIBILITIES:
1. Assesses the health care needs of residents and works with the physician, where appropriate, in developing and implementing a comprehensive plan of care/plan of treatment consistent with professional standards and Seabury policies.
2. Provides direct nursing care consistent with Seabury policies and physician orders under the supervision of a RN.
3. Maintains a current treatment plan/plan of care, updated physicians orders and accurate records on each resident. Communicates a plan of care and instructions to Certified Nursing Assistants. Documents according to Seabury’s guidelines.
4. Functions as a primary nurse, as assigned, in coordinating resident’s needs. Identifies community resources and assists residents/caregivers in accessing services needed for continuity of care. Assists in identifying and ordering supplies/medications/equipment needed for patient care.
5. Supervises Certified Nursing Assistants in performance of their resident care responsibilities.
6. Provides consultation, instruction, orientation and in-services education to other members of the healthcare team, where appropriate.
7. Involves residents and their families/significant others in developing and implementing the plan of care.
8. Provides resident and family teaching including (but not limited to) teaching regarding safety, advances directives, patient’s rights, medications, and specific therapists.
9. Maintains professional licensure and other requirements (i.e. CEU credits) as required by all states in which the LPN will practice.
10. Demonstrates compliance with all of Seabury’s policies and procedures, including residents care practice, clinical record documentation and educational commitments.
11. Reports resident progress to case managers, physicians, clinical managers and other team members as appropriate. Documents coordination of care and communication with other disciplines.
12. Participates in discharge planning and discharge teaching. Provides discharge documentation at time of discharge.
13. IV NURSES ONLY: Performs IV procedures and therapies according to the guidelines set in Seabury’s IV program.
REQUIRED QUALIFICATIONS:
· Currently licensed as a Licensed Practical Nurse in the state of Connecticut
· IV LPN: Previous experience in IV therapy. Completes PharMerica’s IV program or shows proof of competency through an IV Certification Course.
· One year of acute care experience required preferred.
