Social Service Director
Summary: The Social Service Director is responsible for identifying the medically related psychosocial needs of residents and providing services to meet those needs. The Social Service Director will address residents’ intellectual, developmental, emotional, spiritual and social functioning so that each resident can attain or maintain the highest level of psycho-social functioning and well-being possible.

Professional Qualifications:
In facilities of more than one hundred twenty (120) beds, the facility must employ, full time, a qualified social worker. A qualified social worker is one (1) of the following:
(1) Indiana board certification in social work under IC 25-23.6-5-1 with at least one
(1) years’ experience in a health care setting working directly with individuals.
(2) An individual with a bachelor’s or advanced degree, or both, in social work or a
bachelor’s or advanced degree, or both, in a human services field, including, but not
limited to:
(A) sociology;
(B) special education;
(C) rehabilitation counseling;
(D) psychology; and
(E) gerontology;
and one (1) year of supervised social service experience in a health care setting working directly with individuals.
In facilities of one hundred twenty (120) beds or less, a person who provides services is an individual with one (1) of the following qualifications:
(1) Indiana board certification in social work under IC 25-23.6-5-1 with at least one
(1) years’ experience in a health care setting working directly with individuals.
(2) A bachelor’s or advanced degree, or both, in social work or a degree in human
services fields, including, but not limited to:
(A) sociology;
(B) special education;
(C) rehabilitation counseling;
(D) psychology; and
(E) gerontology;
and one (1) year of supervised social service experience under the supervision of a qualified social worker in a health care setting working directly with individuals.
(3) A high school diploma or its equivalent who has satisfactorily completed, or will
complete within six (6) months, a forty-eight (48) hour social service course approved by the division. Consultation must be provided by a person who meets the qualifications under subdivision (1) or (2). Consultant by a person who meets the qualification under subdivision (1) or (2) must occur no less than an average of four (4) hours per month.
(4) Ordained minister, priest, rabbi, or sister or brother of religious institutes who has
satisfactorily completed a forty-eight (48) hour social service course approved by the division. A person who has not completed a course must have consultation of no less than an average of four (4) hours per month from a person who meets the qualifications of subdivision (1) or (2) until the person has satisfactorily completed the division approved course.

Essential Job Functions:
1. Assist, as directed, in pre-admission process which may include involvement in pre-screening, tours and coordination of information and referrals from community members and agencies.
2. Provide social services in accordance with accepted facility, professional and governmental regulatory standards.
3. Act as a resident advocate and liaison with facility staff and with community resources as necessary.
4. Maintain social service records including initial psycho-social assessments, ongoing pertinent progress notes, quarterly notes and discharge plans, documenting in accordance with accepted facility, professional and governmental regulatory standards.
5. Participate in comprehensive resident care planning.
6. Participate in completion of the social service section of the MDS to ensure accurate assessment is completed and supportive documentation is present.
7. Participate and assist with the Resident Council meetings.
8. Responsible for discharge planning and assisting families in placement arrangements, home care referrals, conversion to alternate payment sources, etc.
9. Ensure residents have necessary personal care items, either by notifying families or shopping for residents.
10. Communicate with government agencies, community resources, etc., regarding resident needs or problems (e.g., Medicaid applications, social security questions and arrangements, follow up on discharge, legal aids, private vendors, vocational rehabilitation, scheduled appointments, etc.)
11. May give or assist in giving in-service programs to staff regarding:
a) Psycho-social needs of the elderly and institutionalized resident,
b) Communication with hearing, vision, or speech impaired,
c) Overview of community resources,
d) Social Service needs and resources,
e) Death and dying,
f) Behavior management,
g) Use of psychotropic medications in behavior management, and
h) others on request.
12. Respond promptly to notification that a resident has a social service concern warranting intervention.
13. Remain actively involved in ensuring residents are placed relative to physical, mental and psycho-social needs.
14. Monitor residents at risk for social isolation and/or depression and provide individual programming as necessary.
15. Assist in routine evaluation of psychoactive medication use and efficacy.
16. Develop, as needed, and participate in the implementation of an appropriate Behavior Management Program for applicable residents.
17. Review psychological consult findings and/or developmental screenings ensuring any recommendations therein are addressed accordingly.
18. If applicable, on a daily basis, assign work to a subordinate, schedule lunch and rest breaks, authorize early departures from work, accommodate coverage for absenteeism, assign overtime, reassign employee from one work area to another. Prepare written evaluations of job performance (probationary or annual) which has direct impact on wage increases. Maintain authority to issue verbal and written disciplinary action enforcing work rules, monitors job performance and give instructions as to correct performance. Interview applicants for assisting in social services and recommend hire, direct the work of the subordinate, effectively recommend discipline or discharge.
19. Be familiar with procedures and policies of the facility.
20. Assist and encourage residents in social interaction with other residents, family and staff.
21. Promote facility-community interaction.
22. Ensure confidentiality of resident information.
23. Serve on committees as designated by the Administrator.
24. Perform other related duties as assigned.
Physical, Sensory, and Communication Requirements:
1. Ability to read and write in English, ability to communicate with residents, families, personnel and support agencies and ability to remain calm under stress.
2. Bending, lifting, walking, grasping, fine hand coordination, ability to distinguish smells, tastes and temperatures, ability to discern colors, ability to hear and respond to pages.
