Job Summary
Facilitators that are responsible for helping to identify issues that may threaten the safety of children at home, address the status of behaviors or conditions that present risk of abuse or maltreatment to the child, and to help family members create a plan that will address them, while promoting the advancement of a permanency and/or stabilization plan for the child/family. Utilize a strength-based perspective in guiding the processes that is solution focused. Maintain a neutral and/or objective perspective while guiding the problem-solving solutions raised by staff, children, and/or family members.

Job Summary

Duties And Responsibilities
- Facilitates, attends, participates, coordinates, and monitors internal and external meetings as relevant to program needs.
- Responsible for participating of Clinical Diagnostic Team Meeting to ensure goals and outcomes identified for the service plan are incorporated appropriately
- Facilitates, attends, participates, coordinates, and monitors internal and external training as relevant to the staffing and program needs.
- Responsible for ensuring the notification and coordination with ACS, FSU worker, Mental Health, Health, Substance Use providers
- Works with the Preventive Program to ensure family team conferences are scheduled for families.
- Responsible for preparation for all Preventive family team conferences held at SSCF and/or in the community.
- Provides ongoing education to families on family team conferencing model.
- Responsible for case documentation and data collection.
- Coordinate training for program staff on FTC model.
- Manage all QI reports associated with FTC’s.
- Responsible for timely documentation the case outcome of the Family Team Conference in CNNX.
- Ensure that the solutions and/or next steps are measurable, have time limits and are matched to needs by identifying whom, what and when to accomplish next steps.
- Assure all stakeholders have the FTC summary report, which is inclusive of next steps, timeframes and any referral sources.
- Submit weekly calendar to Unit supervisor/Program director to reflect all FTC’s will be facilitated.
- Flexibility in work schedule to assure FTC’s occur internally/externally.
- Complete other assignments as needed or designated by the Program Director.
Skills, Knowledge, and Abilities:
- Computer proficiency.
- Excellent verbal written, organizational, communication and interpersonal skills.
- Ability to perform the essential functions of the job with or without a reasonable accommodation.

- Thoroughly completes all assigned work within the prescribed timeframes.

- Bachelors with 4-5 years’ experience; MSW preferred.
- Bi-Lingual Spanish, Chinese/Mandarin, Russian, Italian, French Creole is preferred.
- Due to the nature of the work environment work hours will be based on needs of program and may vary.
- Our Agency operates in 2 locations: Staten Island and Brooklyn. In rare occasions, you may be temporarily transferred to either site to meet program needs.
- Performs other duties/responsibilities as assigned within the scope of position.
